PREFACE
For centuries infectious diseases have threatened mankind, and on many occasions they
have changed history profoundly. Today, infectious diseases continue to pose enormous
challenges to human well being and medical science worldwide. While antibiotics, novel
therapeutics, vaccines, new genetic analyses, and sensitive diagnostic procedures have
improved our ability to prevent, treat, and control infectious diseases, their elimination will
occur only through extensive, collaborative national and international biomedical and
public health research.
The University Hospital for Infectious Diseases (UHID) is the leading infectious disease
clinical and research facility in Croatia and the region. Its infectious disease focus is
unique and provides the ability to manage patients harboring a wide variety of pathogens,
conduct research with modern diagnostic and analytical tools, teach undergraduate
students, and train postgraduate nursing and clinical specialists. This brochure is intended
to briefly inform readers about the members and activities of the research groups at the
UHID. The experts at UHID would be honored to discuss potential collaborations on
infectious disease challenges with colleagues worldwide.
Recently, thanks to the support of the Croatian Ministry of Sciences, Education and
Sports, a Central Diagnostic Laboratory and the first Croatian Biosafety Level 3 and
Biosafety Level 2+ Laboratory were constructed at the UHID. These laboratories will
facilitate the involvement of UHID scientists in collaborative projects. In addition, the
Ministry has supported the employment of 14 new PhD students at the Infectious Disease
Clinic, guaranteeing future clinical and research expertise.
In keeping with the longstanding mission of the UHID, its experts participate in many
national and international projects. This brochure describes some of the clinical and basic
research activities of UHID scientists in different areas including: Nosocomial Infections,
Endocarditis, HIV/AIDS, Hepatitis B,C,E, Papillomaviruses, Sexually Transmitted
Infections, Urinary Tract Infections, Respiratory Tract Infections (emphasis on Flu),
Antimicrobial Therapy and Resistance, Pediatric Infections / Diseases, Infections in the
Immunocompromised, Imunopathogenesis of Infections, Emerging and Re-emerging
Infectious Diseases, and Tropical / Parasitic diseases.
Modern UHID infectious disease research will build on a tradition of success. For example,

The University Hospital for Infectious Diseases (UHID) is affiliated with the University of
Zagreb Medical School and is the leading institution for the treatment of infectious
diseases in Croatia. This tertiary care provider treats over 8,000 inpatients and 65,000
outpatients per year. Most patients are from Zagreb, but many are referred to the UHID
from throughout Croatia and neighboring countries.
History
The University Hospital for Infectious Diseases was founded in the Spring of 1893 as The
City Hospital for Contagious Diseases (CHCD). The impetus for the facility was the
increase of rapid travel in the late 19th century and the increased the risk for disease
transmission associated with the movement of people and goods. For example, the spread
of cholera through Europe and the Middle East presented great dangers to Croatia and its
neighbors. Gradually, the CHCD grew larger and admitted patients with classical infectious
diseases (diphtheria, scarlet fever, typhus exanthematicus, etc.). During the First World
War, the CHID had 800 beds, and served as civilian and a military hospital. In 1914, the
CHID had a primitive laboratory for the basic analysis of blood and urine. However, thanks
to the work of two distinguished clinicians, Dr. Steinhardt and Dr. Fran Mihaljević, the
UHID grew rapidly between 1920-30. (The UHID was named after Dr. Mihaljević in 1978).
In the 1950s hospital buildings were reconstructed and new buildings and wards were
established, including the first intensive care unit in Croatia. The UHID hospital became
affiliated with the Medical School of theUniversity of Zagreb in 1956 with the establishment
of the Department for Infectious Diseases. This Department has trained undergraduates,
postgraduates and medical students for more than fifty years. Laboratory and diagnostic
medicine capabilities developed rapidly in the 1970 and now include modern immunologic
and molecular medicine procedures. Rapid advances in research programs in many areas
of infectious disease have occurred in the past 20 years.

Research topics
1. Antimicrobial and other adjunctive therapy of critically ill infectious disease patients
a. Hypothermia in patients with bacterial and viral meningoencephalitis
Patients with low GCS and reduced CO2 reactivity measured by transcranial doppler are
included and subdued to hypothermia (33 0C) for 4 days. Measurements of cerebral blood
flow and optic nerve diameter, as well as lactate index are performed daily. Outcome>:
Glasgow Outcome Score
b. Timing of appropriate antibiotic treatment of ICU acquired blood stream infections
Evaluation of outcome of ICU treatment regarding timing of appropriate antimicrobial
treatment particularly in patients with nosocomial blood stream infections.
c. Are glycopetides still the best choice for the treatment of MRSA infections?
Observational study including patients with S. aureus infection. Correlation of outcome
with vancomycin MIC values. Comparison of vancomycin with other antibiotics (linezolid,
daptomycin).
2. Infective Endocarditis
a. Influence of early cardiac surgery on patients’ outcome
Observational study in patients with definite IE – comparison of outcome between early
surgery groups (within 14 days from diagnosis) with no surgery or postponed surgery.
b. Factors associated with development of endocarditis during staphylococcxal blood
stream infection
Observational study including patients with staphylococcal bacteremia. Comparing
patients with endocarditis and no endocarditis regarding cardiac diseases, and permanent
use of statins, acetylsalicylic acid, anticoagulants.

Research topics
Investigation of the causative pathogens, clinical and laboratory parameters, severity of
clinical symptoms and treatment of pneumonia with the development of an algorithm for
diagnosis, assessment of disease severity, hospitalization and antibiotic use. This research is
a continuation of our previous research project.
Novelty in the research of pneumonia is aimed at explaining pathogenic and immunological
mechanisms, occurrence and outcome of disease. Therefore, the focus of our investigation is
the role and the effects of beta-chemokines and matrix metalloproteinases (MMPs) in the
pathogenesis of the disease.
- Beta-chemokines and matrix metalloproteinases most probably participate in complicated
pathogenic and immunologic mechanisms associated with the development and clinical
presentation of pneumonia. Our hypothesis based research aims to determine their roles in
the pathologies associated with organisms that frequently cause community-acquired
pneumonia (Streptococcus pneumoniae, Mycoplasma pneumoniae, Legionella pneumophila)
- In patients with pneumonia we record an elevated level of beta-chemokines (IL-8, MCP-1
and MIP-1β) in systemic circulation
- The level of beta-chemokines and matrix metalloproteinases in systemic circulation has a
very good correlation with the clinical course of disease . The clinical course of disease is
more severe in patients with higher levels of metalloproteinases.
- The values of these immunological indicators can be correlated with routine laboratory
findings, and also with interrelations
Other hypotheses:
- The growing resistance of pneumococcus to beta-lactam antibiotics is not today a barrier for
the treatment of pneumonia with penicillin and other beta-lactam antibiotics
- It is necessary to determine the criteria for severity of disease, hospitalization, when to
switch therapy and the duration of pneumonia treatment

CENTER FOR EMERGING AND RE-EMERGING INFECTIOUS DISEASES
Emerging infectious disases (EID) are those that have recently appeared in the population,
while re-merging diseases (REID) are those that appear again and then spread in the
population. Many of the causative agents are dangerous pathogens, usually resistant to
existing drugs. Most are microorganisms that could be agents of bioterrorism.
The University Hospital for Infectious Diseases "Dr. Fran Mihaljević" together with national
and international partners from Europe and USA have organized into a virutual collaborative
core-center with the goal of developing a miltidisciplinary approach to EID/REID. The national
partners are the Institute of Immunology Zagreb, the Institute of Public Health Brodskoposavska County, Slavonski Brod, the Faculty of Forestry, the Faculty of Veterinarian
Medicine and the Croatian Academy of Sciences and Arts, Zagreb.
Specific aims of the project are:
(1) Diagnostics –The introduction and development of existing and new diagnostic tools for
EID/REID.
(2) Research – Studies on molecular epidemiology, immunopathogenesis, new therapies
and clinical characteristics of EID/REID.
(3) Education – Development and organization of national and international courses and
conferences on EID/REID topics; training of doctoral studies through existing and new
progams; continuous theoretical and practical education for experts and populations at risk in
various national and international frameworks and programs.
