Tap Request or Meter Change Application
All meter tap and change requests are submitted online. The online form ensures that all
necessary information is provided to process the application.

Meter Owner Information:

For Request Type, you have three choices:
•
a. New meter set – a meter does not exist at the site and
you are requesting one.
Required fields are
noted with an
asterisk (*)
b. Increase in volume – there is a meter and you would like to
bring more gas into one of EQT’s pipelines.
c. Meter/plate change– a meter exists but you would like to
have the meter or plate changed.

The Company Name is the name of the company requesting the tap/meter.

The Contact(s) is/are the person(s) that can answer questions about the request.
7.
The Address is where paper correspondence should be sent.
8.
The Phone # is where the contact can be reached. The correct format is to the
right of the field (i.e. no parentheses, dashes or periods).
9.
The Fax # is where to fax correspondence. The correct format is to the right of
the field (i.e. no parentheses, dashes or periods).
10. The E-mail Address is where to send e-mail correspondence.

Tap Information:
11. Enter the Total Volume/Day (Mcf/D). This number is the
total amount of gas, in Mcf, that will flow through the meter.
If the meter already exists, this amount would be the
additional gas plus the originally assigned maximum daily
quantity (MDQ). It should be realistic and reflect the
amount that will flow within six months of turning-in-line.
The meter will be sized according to the request.
Overstating volumes may result in inaccurate
measurement.
Total Volume/Day (Mcf/D) =
Total amount of gas, in Mcf,
that will flow through the
meter.
For an existing meter, the
volume would be the
additional gas plus the MDQ.
12. The Estimated Pressure (#) is the pressure at the wellhead. If you do not know this
information, leave the field blank.
13. The Gas Quality shows the heating value of the gas. Your choices are: (1) < 1100,
(2) >1100 or (3) Coal bed methane.
14. Meter Number or MID displays only if you have selected Increase in
Volume or Meter/Plate Change. Enter the meter number for the applicable site.

Location Information:
15. Choose the State (KY, PA or WV) where the proposed tap will be located.
16. Type in the County.
17. List the Quadrangle of the site.
18. Enter the Township (not required) of the location.
19. Type the Site Name. This is the meter name that EQT will enter into its
measurement system.
20. Please select the District: Big Sandy, Brenton, Equitrans Transmission,
Equitrans Gathering, Kentucky, Madison or Three Rivers.

21. The Latitude and Longitude are GPS (global
positioning system) coordinates that identify the
exact location of the new tap, additional volume or
meter/plate change.

information.
Coordinates should be in degrees with decimals
(e.g. 39.55555), not °, ' and ". Go to http://www.fcc.gov/mb/audio/bickel/DDDMMSSdecimal.html to convert degrees, minutes and seconds to a decimal format*.
22. The Requested Turn-In-Line (TIL) Date should be at least thirty-days from the date
that your request is made. It is not reasonable to list a date that is the same as the
request date. The proper format is mm/dd/yyyy or 12/01/2010.
23. Additional Information is a field for you to tell us additional information that you think
is important to process your request. For example, if you have a permit number, you
can provide it here.
24. Attach Topographic Map is not required. You can attach a map or other document in
.pdf, .jpg, .doc or .xls by selecting Attach a File. Due to e-mail traffic, large files may
not transmit.

Shipper Acknowledgement:
25. The Shipper Acknowledgement box must be checked to confirm your
understanding of the following statement:
I acknowledge that my shipper is required to notify the Transportation &
Exchange department, using the production release/new meter form, that we
have secured a gas purchase agreement. I also understand that the field tie-in
will not be completed until the form is completed and sent to the T&E group.
If your request is approved, your shipper is required to complete the form at:
http://www.eqt.com/docs/xls/NewMeterForm.xls and forward it to the T&E group.

26. When you have filled in all of the information, click on Submit
Request. If you click on Clear Request, your request will be
cleared and you will have to retype and submit the information.

27. When the request is submitted correctly, the information will be e-mailed to you at the
address provided in the application (see example below). If you do not receive this
email, you have not submitted the request. Please keep the email for your records.

28. If any of the required fields are blank or you do not use the right format, you will see
an error message.
Example: If you use the degrees, minutes and seconds format (°, ' and ") instead of
degrees with decimals (39.11111 Latitude and 80.22222 Longitude), you will receive
the message below.
Until the form is completed correctly, your request will not submit.

Frequently Asked Questions (FAQs) About Meter Taps:
Application FAQs
Q:
What should I do if I don’t have access to the Internet?
A:
Applications should be submitted through EQT’s website. This online form guarantees
that the necessary information is listed. It provides a receipt, with all of the submitted
information, and a date stamp to show that your request was sent. Applications,
submitted using this format, are processed in a timely manner.
Q:
Can I submit for a volume increase or meter/plate change if I don’t know the
meter number (MID) and GPS coordinates?
A:
Even if the meter is already on the system, this information is required. Your
meter number can be found on the monthly shipper statement.
Q:
I have an existing EQT meter and want to drill more wells. Do I have to
apply for this additional gas?
A:
Yes. If you plan to bring on additional gas, you should apply, prior to drilling. The
request will be reviewed in the same way as a new meter tap request. If additional
gas flows without EQT’s approval, your meter may be cut back to the MDQ or
shut-in indefinitely.
Q:
I have an old EQT meter site that is shut-in and I plan on reworking the
well(s) behind it. Do I have to submit a request?
A:
Yes. If there hasn’t been active production for a year or more, you will need to apply
for an increase in volume. The request will be reviewed in the same way as a new
meter tap request.
Q:
Why won’t my application submit?
A:
Typically, you can’t submit the request because one or more fields are not completed
and/or are not in the right format. The most common mistake is that the GPS
coordinates are not submitted in degrees with decimals (e. g. 39.55555) Instead,
customers enter °, ' and ". Your application will not submit if you don’t use the decimal
degrees format.
In a few instances, customers are unable to submit a request because they are using
an older web browser. If you upgrade your browser, you should not run into this issue.

A:

Process & Timing FAQs
Q:
How long does it take to process an application?
A:
Our goal is to process a typical application over thirty business days. More complex
requests may be processed over a longer time period.
Q:
What happens after I submit a request?
A:
When the request is submitted, EQT staff reviews the application from distinct
perspectives, including the Commercial, Pipeline Engineering and Field Operation
groups. When all of the analysis is gathered, a final decision is made and the
customer is notified.
Q:
I spoke to an EQT employee and they said it was okay. Does that mean my
request is approved?
A:
Each department looks at the request from a different viewpoint. It may look
good from one angle and bad from another. Your request is approved only if you
receive written notification of the approval.
Location FAQs
Q:
I don’t know what the GPS coordinates are. Will you still process my request?
A:
GPS coordinates allow us to pinpoint, to a high degree of accuracy, the location of
your proposed meter site. If we do not have this information, the application cannot be
processed.
Q:
Where can I buy a global positioning unit?
A:
Global positioning units can be found at most electronic stores.

Q:
Do you need the topographic map?
A:
The topographic map is no longer required but you can include it with your
application. If the file is large, you may be unable to submit it.
Shipper/Purchaser FAQs
Q:
Do you have recommendations on whom to sell the gas to?
A:
We cannot recommend any shippers. The only requirement is that they have a
contract on the EQT transmission or gathering system that you’d like to tap into.
Q:
Will you buy the gas?
A:
The Commercial group does not buy or sell gas.
Q:
Can I provide the shipper information?
A:
If your company has a fully-executed transportation contract on the applicable
pipeline and will be shipping this gas, yes. If you do not, your shipper is required to
notify Producer Services.
Q:
Will you approve my application if my shipper doesn’t provide a confirmation of
a gas purchase agreement?
A:
When approving requests, customers are mailed an approval packet. Included in this
mailing is the shipper form. For EQT to begin construction on an approved site, the
shipper form must be received by T&E.
