Prescription Collection Pharmacy Ordering How to Order your Some pharmacies will also offer to order your prescription for you. Repeat Prescription You will need to sign a consent form available from the pharmacy if you would like to order in this way. Many pharmacies now offer a prescription collection service. They will collect your prescription for you so that you can collect it directly from them or in some circumstances they will deliver it for you. If you would like the pharmacy to collect your prescription, please tell us when you place your prescription order. Do this by:  adding a note to the request or if ordering online,  enter a note in the comments box detailing the name and address of the pharmacy. Please be aware that you will still need to contact the pharmacy to tell them what you require. Please ensure the pharmacy has the most up to date medication list to ensure you receive the correct medication. You can use this list to reorder your medicines when you run out. Or: Tick the items you require. Post to your surgery  Remember to only order the items you need.  Do not stockpile medications at home.  If you have stopped taking a medication because of side effects or because you feel you no longer need it please let us know as we may be able to prescribe an alternative.   Please tell us if you are taking herbal remedies or items that you have purchased over the counter or from other sources. We need to be aware of all the medication you are taking. If you have access to the internet you may order your prescription online. Remember to include a stamped self addressed envelope if you would like your prescription posting back to you. You will need to register to use our online services by going to our website. www.boltoncommunitypractice.nhs.uk  Go to the prescriptions page and select your branch.  You will be taken to the branch page  Click enrol for online services and submit your details.  You will receive a confirmation email which will include your username. Keep a record of this.  Click the repeat prescription link and enter your username and password.  Enter details of the prescriptions you require. You will receive an email when your request has been processed.  If we are unable to process your prescription in this way we will let you know by return email. Or: Fax your request to the surgery Write down your name, address and date of birth and the items you require. Remember to include the dose you are taking .
