Physician prescribes Voraxaze® Pharmacist/ Physician requests access to Voraxaze® via Clinigen Process 07741242858 Health care institute receives product · · For an order to be processed, the following criteria must be met: 1) Patient requiring immediate shipment of Voraxaze®. 2) Completed Patient Access Form. 3) Regulatory approval (if patient is outside of the UK). 4) Signed Purchase Order or email confirmation of price acceptance with PO reference number. Clinigen acknowledges enquiry immediately and sends documentation pack via email which includes all the information and paperwork needed to access the product.
