3. General Layout Plan;
4. Plan elevation (longitudinal & traverse);
5. Piping plan in isometric drawing & detailed plans of foundation and support;
6. Detailed construction and working plans of boilers and pressure vessels;
7. Complete machinery list in tabulated form;
8. Flow sheet if processing manufacturing or currently operating; and
9. Signature and Seal of Professional Mechanical Engineer.

3. Clearance from the Municipal Mayor, Chief of Police, Municipal Judge and NBI;
4. Mining contract/ Exploration Permit of the area;
5. Operating Agreement if the applicant is other than the Tenement Holder/Permittee;
6. Drilling and blasting scheme prepared by a licensed Mining Engineer justifying the
legitimate use of the Explosives;
7. Plan of the explosive magazines;
8. Location map of the explosive magazines;

P 1,000.00 per application
P2,000.00 per man/day with a minimum amount of P6,000.00. Transportation and incidental
expenses shall be borne by the applicant.
