Welcome to Kindergarten!
Dear Parents/Guardians,
Welcome to the Waltham Public Schools and to the Parent Information Center (PIC). All students
entering Kindergarten for the 2012-2013 school year will be registered at the Parent Information
Center (PIC).
In this booklet, you will find necessary information to register your child for Kindergarten; including
registration details, requirements and schedules, directions and school information.
Kindergarten registration will begin on January 30, 2012 at the PIC. We are located at 30 Parmenter
Road in Waltham. Our office is located at Door #6, down the ramp. Please look for our signs.
Please call the PIC at 781-314-5672 or 781-314-5719 to set up your child’s registration appointment.
Translation services are available in Spanish. To request an interpreter, please call 781-314-5662 or
781-314-5524.
Your child’s Kindergarten screening will be scheduled at the time of registration. Screenings will
occur at the school your child will attend.

3. Bring all information necessary to complete registration forms and meet student registration requirements

Kindergarten Screenings at Elementary Schools
The purpose of screening is to identify the educational needs of each child. Screening includes language, articulation,
fine and gross motor skills, and visual and auditory memory. More information will be given at registration.
May/June 2012
Kindergarten Visitation Days at Elementary Schools
Dates will be available at registration.

Kindergarten Registration Details
At registration appointments, parents will complete registration forms and present the
mandatory documents listed below. Please bring all necessary information.
Please note: Registrations will be considered incomplete until all documentation is provided.
Registrations will not be accepted over the phone.
Age Requirement
To enroll in Kindergarten, a child must be 5 years of age on or before September 1, 2012.
Mandatory Documents
1. Proof of Birth
An ORIGINAL Birth Certificate or Passport is needed to verify a student’s date of birth. A registration cannot be accepted unless authentic evidence of the date of birth is submitted at the time of registration.
2. Immunization Records
The School Immunization Law is specific and requires written documentation from a physician (or
parent in the case of religious exemption) that children have been successfully immunized (or exempted for health reasons) in accordance with the recommended age-appropriate immunization
schedule of the state Department of Public Health.
Required Immunizations for Kindergarten Registration include:

Hepatitis B—3 doses; DTP—5 doses; Polio—4 doses; MMR-2 doses
•
Vision-Screening Exam with Stereopsis (vision testing) Reading and Results
•
Lead test (blood test) Reading and Results
•
Varicella (chicken pox)—2 doses or record of when child had chicken pox.
3. Physical Examination
State law requires each entering child to have a physical examination within six months prior to entrance into Kindergarten (that is within six months prior to September 1, 2012). However, due to
scheduling issues and health insurance regulations this is not always possible. Therefore we will accept a physical exam in the past twelve months. As soon as you get an updated physical, please mail
it to the Parent Information Center (PIC). It is recommended that this examination be done by your
private physician.
**Please note: If using the attached MA Dept. of Public Health Forms, you must have a Doctor’s signature and/or
Doctor’s address stamp. All other records must be on Doctor’s letterhead.**
4. Residence and Guardianship
Students must be legal residents of the City of Waltham and proof of parenthood or legal guardianship must be presented in order for a student to enroll in Waltham Public Schools. Parents are usually listed on birth certificates. Guardians must present Court documentation. Parents/guardians
must also present proof of Waltham residence. Please see below for accepted documentation.

The PIC is closed for registration when Waltham Public Schools cancels school due to snow.
Registration appointments will be rescheduled.
If you will be moving to Waltham after March 23rd, please call the PIC to schedule a Kindergarten registration appointment.

Parking:
Parking for the Parent Information Center is available in the school parking lot in the labeled
“Visitors Parking” spots. In addition, you may also park in nearby permitted street parking spots.
There is NO PARKING in front of the building on Parmenter Road.
Directions:
From Main Street in Waltham:
Turn onto Moody Street and take a left onto High Street. Follow High Street through two sets of
lights and then take a right onto Parmenter Road. The school will be on your left.
Public Transportation:
MBTA Bus Stops near the Parent Information Center include: Crescent Street opposite Cherry Street
on Route 558; Moody Street at Ash Street on Routes 505, 553, and 554; Moody Street at Maple Street
on Route 556; and Moody Street at High Street on Route 170. Detailed routes and schedules can be

School Resources
Transportation:
The school bus schedule is available online and at each school.
Bus routes are updated each August and available to the public
via the Waltham News Tribune, the school website and at each
school. Please contact the PIC at 781-314-5672 for a copy of the
current bus schedule.
To request a change of bus stop or route, you must speak with
your school principal first. Students living 8/10ths of a mile or
more from school are eligible for a bus.
After-School:
There are after school programs available for each elementary school. These programs are managed
by outside organizations and applications are sent home in the first few weeks of school. The
Waltham Youth Activities Listing contains after school program information as well as other after
