2. An official High School Transcript including SAT and/or ACT Scores and a list of Senior
Courses
3. Proof of Registration for Fall 2012.
4. A recent passport or wallet-sized photograph.

2. A final official High School Transcript including SAT and/or ACT Scores verifying date of
graduation
3. A College Transcript including Proof of Registration for Fall 2012.
4. A recent passport or wallet-sized photograph.
5. Additional information as required for each named/memorial scholarship

SERB NATIONAL FEDERATION 2012 Scholarship Requirements
The SNF will offer a total of 22 scholarships this year: 15 SNF Scholarships plus 7 named/memorial scholarships. The 15 SNF Scholarships will be paid at $250 per year for four years of enrollment. Awardees will be notified by the Serb National Federation, and the scholarships will be announced in the American Srbobran, the official organ of the Serb National Federation. REQUIREMENTS FOR ALL APPLICANTS: 1. Deadline for completed applications is FRIDAY, JUNE 15, 2012. 2. Each applicant must submit a 2012 Scholarship Application; an official high school transcript; S AT or ACT scores; proof of college registration; a college transcript, if applicable, and a recent passport or wallet-‐sized photograph. If applying for the s eparate named/memorial scholarships please include additional information required for each. 3. All applicants must currently be a member of the SNF and must have joined prior to January 1, 2010. Membership must b e continued during the four-‐year p eriod for all scholarship recipients. 2012 Named/Memorial Scholarships The Serb National Federation will award 7 separate named/memorial scholarships in addition to the SNF scholarships. Awardees will be notified by the Serb National Federation and the scholarships will be announced in the American Srbobran, the official organ of the Serb National Federation. These scholarships each have a value of $1,000. Applicants may reapply annually but will be judged by the Scholarship Committee as if an initial application. REQUIREMENTS FOR ALL APPLICANTS: 1. Deadline for completed applications is FRIDAY, JUNE 15, 2012. 2. Each applicant must submit a 2012 Scholarship Application; an official high school transcript; S AT or ACT scores; proof of college registration; a college transcript, if applicable, and a recent passport or wallet-‐sized photograph. For the separate named/memorial scholarships, p lease include additional information required for each. 3. Applicant must present a letter of recommendation from a priest describing church involvement (Sunday School, Altar Boy, Church Choir, etc.). +Gustav and Milana K arlo Bizic Scholarship (1 Award) Additional Requirements: 1. Applicant must have successfully completed two ( 2) semesters of h is/her undergraduate program from an accredited d egree-‐granting college or university. 2. Applicant must present a letter of interest to the SNF Scholarship Committee d emonstrating the applicant’s qualifications, including official transcripts from a ll undergraduate or graduate schools attended. 3. Applicant must submit a current resume. 4. Applicant must submit two (2) letters of recommendation, at least one from a p erson qualified to verify the applicant’s interest and commitment to the Serbian community. Sally Davis Memorial Scholarship (1 Award) Additional Requirements: 1. Applicant must b e currently enrolled as a s econd year student or beyond in an accredited college or university. 2. Applicant must present a letter of a pplication to the SNF Scholarship Committee which includes the level and field of study and transcripts from each college or university already attended. 3. Applicant must present a letter of recommendation from a priest describing church involvement (Sunday School, Altar Boy, Church Choir, etc.). Ann Davis Memorial Scholarship (1 Award) Additional Requirements: 1. Applicant must b e currently enrolled as a s econd year student or beyond in an accredited college or university. 2.
