Aufgabe und Wirkung

Das ISTclassic® NE -Gerät nach Prof. Hinz hat
zwei Aufgaben:
1. den Unterkiefer während des Schlafens in einer
Protrusionsstellung zu fixieren
und
2. gleichzeitig die Zunge, den Zungengrund und
die angrenzenden Weichteile mit nach vorne zu
verlagern
Dadurch wird:
eine Vergrößerung des hinteren Rachenraumes
erreicht
der Verschluss oder eine Einengung des hinteren Atemweges durch eine kollabierte Zunge
vermieden

Indiziert ist ein ISTclassic® NE -Gerät, um:
primäres – harmloses – Schnarchen zu vermindern oder zu vermeiden
bei leichtem obstruktivem SchlafapnoeSyndrom zeitweiligen Atemstillstand zu verhindern
Grundsätzlich sollte vor der Anwendung eines

ISTclassic ® NE -Gerätes seine Indikation durch
eine qualifizierte Befunderhebung und Diagnose
in einem ambulanten oder klinischen Schlaflabor
festgestellt werden.

Die Befestigungen der Vorschubstege wurden bei den neuen IST-Geräten anterior
in die Oberkiefer- und dorsal in die Unterkieferschiene gelegt. Die geänderten
Drehpunkte bewirken bei Mundöffnung eine weitere Vorverlagerung des Unterkiefers, so dass eine Einengung der oberen Atemwege – im Gegensatz zu allen
anderen Apparaturen, die nach dem Prinzip des Herbstgeschiebes konstruiert
sind – nicht erfolgt. Die intermaxillären Gummizüge, die den Tragekomfort
einschränken, sind entbehrlich. 50% des maximalen Vorschubs ist in der Regel
ausreichend und kann bei Bedarf stufenlos adjustiert werden.
Das neue Laminierverfahren nach Prof. Hinz macht die Schienen nahezu unzerbrechlich
und farbstabil. Unsere IST-Konstruktionen lassen sich darüber hinaus mit allen
anderen Tiefziehverfahren verarbeiten.
Function and indication

The ISTclassic® NE -appliance from Prof. Hinz
has two functions:
1. fixing the lower jaw in a protruded position
during sleep
and
2. repositioning tongue, tongue base and adjacent soft tissue forward at the same time
This will:
enlarge the posterior pharyngeal space
prevent the posterior airway from occlusion or
obstruction by a collapsed tongue

The ISTclassic® NE-appliance is designed to:
reduce or avoid primary – harmless – snoring
prevent temporary respiratory halt in mild
obstructive sleep apnea syndrome

Generally the indication of an ISTclassic ® NE-appliance should be assessed by a qualified anamnesis and diagnosis in an ambulant or clinical sleep
laboratory.
The fixtures of the protrusion elements in the new IST-appliances was
placed anteriorly in the upper and dorsally in the lower splint.The altered
rotation point causes further advancement of the lower jaw in an open
mouth position so that obstruction of the upper airway is avoided – contrary to all other appliances designed according to the herbst principle.
Intermaxillary elastics, reducing patient comfort – are dispensable. 50 %
of maximum protrusion is usually enough and can, if required, be adjusted continuously.
With the new laminating procedure from Prof. Hinz the splints are nearly unbreakable and stable in colour. Our IST-constructions can also be used with
all other pressure forming procedures.

Modellvorbereitung zum Tiefziehverfahren / Preparation of the cast models before pressure forming
Mit Hilfe des Parallelometers wird der
Äquator der einzelnen Zähne bestimmt
und angezeichnet. Die Zahnzwischenräume und unter sich gehende Stellen
werden mit Wachs ausgeblockt (nur
möglich beim Laminierverfahren).
Das Modell wird in Füllgranulat eingebettet und mit Isolierfolie tiefgezogen.
A parallelometer is used to identify
and mark the equators of the teeth.
Interdental spaces and below-equator
areas are blocked out with wax (only
possible with laminating procedure).
The model is embedded in fill granulate and the separating foil is pressure
formed.
Die Isolierfolie wird mit dem Skalpell bis
zum Modellrand gekürzt. Die Isolierfolie
bleibt auf dem Modell. Die LAMIone-Folie
wird darüber tiefgezogen. Der Überschuss
wird grob bis in die Umschlagfalte und

Die Steghalterungen werden nach Anpassung der Vorschubstege mit Acrylat
auf der OK-Schiene befestigt und im
Drucktopf polymerisiert.
Die Steghalterungen werden auf der
LAMIone-Folie im UK im Bereich der Molaren angepasst und mit elastischem
Material fixiert.
Die Steghalterungen werden nach Anpassung der Vorschubstege mit Acrylat
auf der UK-Schiene befestigt und im
Drucktopf polymerisiert.

Die vorgetrocknete LAMItwo-Folie wird
in das Tiefziehgerät mit der weichen
Folienseite nach oben eingespannt.
Die Folie wird mit dem Haftvermittlertuch unmittelbar vor der Erwärmung
konditioniert.

Die laminierten Folien dürfen erst jetzt
vom Modell abgehoben und auf ihre
endgültige Länge gekürzt werden.
Die fertigen Schienen sollten ca.
1-2 mm über dem angezeichneten
Äquator liegen, um einen optimalen
Halt zu bieten. Zu straff sitzende
Schienen lassen sich leicht kürzen –
aber nicht mehr verlängern.

Die Kürzung der tiefgezogenen Folien und
die endgültige Schienenrandlänge der
laminierten Schienen wird zweckmäßig
mit einer dreieckigen Schneidefräse* (gelbe
Markierung) vorgenommen. Die Glättung

Schienenränder erfolgt mit Ausarbeitungsscheiben.

Die Wirksamkeit des IST-Gerätes ist bei
primärem Schnarchen sowie bei
leichtem bis mittelschwerem Schlafapnoe-Syndrom wissenschaftlich belegt.
The efficacy of the IST appliance has
been scientifically proven for cases of
primary snoring and a mild to moderately
heavy sleep apnea syndrome.
Bisher gibt es jedoch keine prädiktiven Parameter, die einen absoluten Behandlungserfolg bei jedem Patienten voraussagen.
Mit dem IST-Testgerät nach Prof. Hinz kann
vor einer Behandlung – polysomnografisch

überprüft – die Indikation für ein kostenaufwändiges IST-Gerät gestellt werden.
So far there have been no predictive parameters that can forecast a complete success for every patient. The IST test appliance from Prof. Hinz can be used prior to
treatment (polysomnographically tested) to
determine whether there is an indication for
an expensive IST appliance.
Das IST-Testgerät ist aus einem biokompatiblen thermoplastischen Kunststoff hergestellt. Ein idealer Zahnbogen mit Aussparungen für die unterschiedlich großen Frontund Seitenzähne erleichtert die Anpassung.
Die Lage des Unterkiefers ist um 6 bis 7 mm
protrusiv vorgegeben.
Den Bisslagen KL. I und KL. II wurde durch
zwei Gerätetypen entsprochen.
The IST test appliance consists of biocompatible thermoplastic material. An ideal dental arch with recesses for anterior and lateral
teeth of different sizes facilitate adjustment.
The position of the lower jaw is fixed with a
6 to 7 mm protrusion.
Two different appliances are available for
the Class I and II bites.
