How to Find a Provider

Blue Cross and Blue Shield offers a large broad nationwide
network of providers. Take a look to see who’s included in your
area by using the online tool:
To find a dentist in the Delta
Dental PPO Network:
Go to www.deltadental.com.
1. Go to www.bcbs.com.
Click on “Dentist Search.”
2. Click on “Find a doctor or hospital.”
Choose “Delta Dental PPO.”
3. Select a Specialty, or leave blank.
Then, enter the additional
search criteria.
4. Select a search distance and enter your city/state or
zip code.
5. Click on “Go.”
To find a dentist in the Delta
Premier Network:
6. Search results provide you a list of our network providers.
Go to www.deltadental.com.
You can also call Blue Cross and Blue Shield at 1-877-258-3760
and a representative can help you find a network provider.
Representatives are available Monday – Thursday from
7:00 a.m. to 8:00 p.m. Central time and Friday from 9:00 a.m.
to 8:00 p.m. Central time.
Click on “Dentist Search.”
Choose “Delta Dental Premier.”
Then, enter the additional search criteria.
Vision Plan
To find an eye doctor in the
EyeMed Vision Plan network:
1. Log on to www.ybr.com/landolakes.
2. Click on “Health and Insurance”
in the top toolbar.
3. Under “Plan Details”, click
on “Vision.”
4. Click on “Find an
Eye Doctor.”
