Human Service Levy History
In the fall of 2003
2003, the passage of the
Human Service Levy contained one
additional mil to help address the needs of
vulnerable residents.
The increase in levy funding provides
about 5.6 million dollars per year to help
the frail elderly residents age 60+ in
Montgomery County.
ComCare
ComCare is a program supported by
Human Services Levy that provides a
package of inin-home services to individuals
who are frail and elderly and need support
to remain in their homes
homes.
This program is administered by the local
Area Agency on Aging
Aging.
ComCare
Care managers work with consumers and
their caregivers to develop an
individualized care plan to promote
independent living.
Contracted providers provide in
in--home
services based on the assessed need and
care authorization
authorization.
ComCare Services
The menu of services available include:

- home delivered meals

History of ComCare
ComCare was developed 10 years ago to
address the growing number of frail elderly
who needed help to stay at home but
could not meet the strict eligibility
requirements of the PASSPORT program.

has occurred in ComCare, there are about
300 frail elderly individuals waiting for
services.
