acksonville Orthopaedic Institute is expanding to meet
the needs of the communities we serve. We are pleased to
welcome another new physician to our group practice. This
brings our total to 29 physicians practicing in seven convenient
locations in Northeast Florida.

Our physicians work closely with our therapists and athletic trainers at each of
our six state-of-the-art rehabilitation centers. Each facility is designed to meet the
specific needs of our orthopaedic patients. JOI doctors work closely with our staff
to thread a continuum of care throughout rehabilitation in order to facilitate
each patient reaching his or her highest threshold. We offer aquatic therapy, biomechanical analysis, custom splinting, hand therapy, occupational therapy, pre-and
post-op therapy programs, physical therapy, spine and back programs, sport injury
rehabilitation, throwing programs, and work injury rehabilitation. We treat
thousands of patients every year – patients depend on JOI. Site locations include
Beaches, Point Meadows, Riverside/Westside, San Marco, South, and University.
Our mission is to meet the orthopaedic needs of our patients through diagnosis,
treatment and rehabilitation. This year we are focusing on working even closer with
our region’s primary care physicians to ensure a continuum of care. We are
offering a series of three continuing medical education programs to bring up-to-date
information on orthopaedics to these physicians as we work together to provide
efficient and effective care focused on the positive outcome of each patient. We
believe in a “team” approach! We are also offering education opportunities to
Workers’ Compensation case managers and adjusters in order to share information
on how we hope to get our patients back to work as quickly and safely as possible.
We soon begin our 13th year “on the field” of many schools, colleges and universities in the area, as well as the Jacksonville Jaguars. JOI is proud to be the “official sports medicine partner” of this great NFL team. Also, we volunteer our expertise to the Jacksonville Sports Medicine Program to provide free pre-participatory
physicals to high school athletes in order to prepare them for the Fall.

Baptist – Beaches, Baptist – South), Memorial Medical Center, and St. Vincent’s
Medical Center. We also work with more than 140 insurance companies to provide

physiatrist is a physician specializing in physical medicine and
rehabilitation. It is the branch of
medicine emphasizing the prevention,
diagnosis, treatment and rehabilitation of
disorders involving the neuromuscular,
cardiovascular, and pulmonary systems.
It is unique among medical fields in that its
area of expertise is the functioning of the
whole patient, as compared with a focus on
an organ system. Our primary goal is to
provide medical care to patients with pain,
weakness, numbness, and loss of function
so that they can maximize their physical,
psychological, social, and vocational
potential.

In my practice, I take care of patients
with pain in the cervical, thoracic and
lumbar regions, numbness in the upper and
lower extremities, and ailments in the knee,
hip and shoulders. A large part of my
practice involves low back pain because of
its high incidence in the general public. As
you might know, eight out of 10 people are
destined for at least one episode of low
back pain over the course of their lifetime.
It is the most common cause of job-related
disability in the U.S., and the most common
neurologic ailment in the U.S., costing
greater than $100 billion per year.
As a specialty, we are extremely aggressive with accurate diagnosis but conservative with the management of care.
A detailed history and physical exam
supplemented with appropriate imaging
studies is routinely executed to achieve this
objective. The treatment for low back pain,
for example, is carried out in a threephased approach.
Phase I involves pharmacologic therapy,
such as non-steroidal anti-inflammatory
drugs (naprosyn, ibuprofen), modalities
such as heat/cold and electrical stimulation,

and physical therapy for core and lumbar
stabilization exercises, postural training
and education.
Phase II includes epidural and facet
joint steroid injections that treat inflammation and help to break the pain cycle. It also
includes radiofrequency procedures which
ablate or deaden the pain-generating nerve.
Phase III involves more advanced
procedures such as IDET (intradiscal
electrothemal) and Nucleoplasty procedures. The IDET procedure delivers
thermal energy through a resistive heating
coil to seal the tear in a disc. The
Nucleoplasty procedure is a minimally
invasive procedure that uses electrothermal
energy to decompress a bulging or
contained herniated disc.
In addition to spine disorders, physiatrists treat a number of sports-related
shoulder, hip and knee injuries such as
rotator cuff and biceps tendinitis, iliotibial
band syndrome, and patellofemoral pain
syndrome. We specialize in optimizing
peak performance especially in highly
competitive athletes. Pre-established treatment protocols, proper nutrition, and
appropriate counseling make this possible.
Our practice also involves electrodiagnostic studies that are used to diagnose
disorders such as carpal tunnel syndrome,
cervical or lumbar radiculopathy, and
peripheral neuropathy. This is a highly
sophisticated test that records electrical
impulses and is composed of two parts: a
nerve conduction study and an electromyographic study. This test can be used to
track the progress of an injured nerve as it
regenerates.

Do You Need
A Personal
Trainer?

HY do you need a personal trainer in the
first place? Isn’t physical fitness and exercise a private affair? For many, the answer
is yes—we’ll get to that later. But more and more
people are opting to enlist the aid of a personal trainer, and they do so for the same reason that someone
who is furnishing a living room might call in an interior decorator for advice.
Having a personal trainer can have several
benefits. First, and most obvious, you get personal
attention. That means the exercise regimen can be tailored to your specific health needs and your personal
goals. Plus, it gives you the advantage of having a
professional who can provide expert guidance.
Compare it to this: How many of us, as kids, learned
to play the piano or guitar all by ourselves? True,
some are self-taught, but many if not most had a ‘personal trainer.’ The same is true with physical fitness.
Whether your goal is to lose weight or just to tone up,
a personal trainer might be able to help you accomplish your goals.
Some find that having a personal trainer gives
them motivation. After all, now they’re accountable to
someone person. They can’t just say, “I’ll start tomor-

row.” And trainers are more than people
who just call out the number of repetitions
you should do. These are people who can
work with your specific agenda. But how
do you find a trainer who’s just right for
you?
Writing in Bottom Line, personal
trainer Joe Sweeney (who is also the
author of a book entitled I Know I Should
Exercise, But …) suggests that you set up
an appointment with your prospective
trainer. In a 20- or 30-minute discussion,
find out his or her area of expertise. What
experience does this trainer have with
your age group and fitness level? “You
should not have to pay for these appointments,” Sweeney says.
There are several other things to
look for in a trainer. For example, what is
his or her certification? If you want quality
assistance, your trainer should have a fouryear degree in some type of fitness-related
field. You can even ask to see a copy of
any certificate he may have. Look into his
liability insurance policy, too, while you’re
at it. Any good trainer will not hesitate to
offer you a written contract, spelling out
all fees and conditions of your program.
Make sure that any trainer you are
thinking of enlisting is prepared to tailor
his workout to your abilities and goals.
“He should start by assessing your health,
nutrition, fitness and medical history,”
writes Joe Sweeney. “He may test your
body-fat percentage, flexibility and muscular strength. The trainer should be a
great listener and take into account what
you hope to gain from your workouts—
weight loss, more stamina, better posture,
etc.”
But a caution is in order. “Not all
trainers are created equal,” says Barrett
Niehus at the web site alltrainers.com. He
recommends: “You should choose a personal trainer using the same criteria that
you use to choose your personal physician.
After all, the personal trainer is a health
care professional that works by preventing
disease. Evaluate the trainer’s background,
their qualifications, their references, and
their ability to communicate and relate to
your needs. You are looking for a profes8

sional that will suit your needs and be able to help
you achieve your goals.”
Of course, personal training will cost. One
program sets the price at about $30 for a half hour
session and $50 for a full hour. Rates are lower if
you sign on for a longer term, perhaps 20, 50, or 75
weeks, and lower still if you share with someone
else—in other words, one trainer to two clients.
Another route is to look into an “on-line”
personal trainer. A number of web sites offer ‘personal’ advice tailored to your personal goals and
information—but it’s all done by e-mail. Not bad—
it’s semi-personal, and it’s only semi-costly! For

example—and this is just one example—for about
$30 per month, Desert Valley Personal Training
offers a personal program every four weeks. “These
programs are completely individualized based on
your goals and health information,” asserts trainer
Ryan Crandall. “I’ll be beside you all the way with
unlimited email support.”
All told, the process of choosing a personal
trainer need not be difficult. “There are hundreds of
extremely qualified and professional personal trainers out there,” writes Barrett Niehus. The choice
merely requires a little homework and something
more than pulling a trainer off the floor of your local

health club. Your personal trainer is your
employee, and you are paying him/her to
help you achieve a goal. If you are willing
to pay money to become fit, why not
choose a professional that will make you
succeed?”
You can start your search for a personal trainer at your local health club, or
you can investigate web sites like freetrainer.com or ustrainersearch.com. Also, check
out the web site of the Personal Trainers
Marketing Association (alltrainers.com).
Do It Yourself?
But can’t you do all this yourself?
As mentioned at the outset, some can. The
challenge is to know which regimen is best
for you and to keep motivated. So if you’re
going to try to be your own personal trainer,
here are some tips from Dr. Miriam Nelson
from Tufts University, as printed in Bottom
Line:
Work out with a friend. If someone else is counting on you to be there,
you’re less likely to miss a workout.
Make a realistic schedule.
Whether you exercise every day for 30 minutes or 30 minutes three times a week,
make sure it’s a schedule that is practical
for you. “If you set unrealistic goals,” Dr.
Nelson says, “you’ll quit.”
Put it in writing. Chart your
progress. Check off each day on your calendar after you’ve completed your exercise.
Better still, keep a detailed exercise log,
including the date that you exercised, how
long you worked out, and what routine you
performed. “Charting your progress will
motivate you to maintain your program,” Dr.
Nelson says.
Don’t pamper yourself. Monitor
whether a routine is too easy, just right, or
just plain too hard. “Exercise won’t do you
much good unless you work out at an appropriate level of intensity,” says Dr. Nelson.
So, just as a personal trainer would push you
to a realistic limit, ask yourself: Could I have
exercised for a longer period? Should I have
included more difficult exercises? Of course,
if your limbs are heavy or sore after a workout, your intensity level might be too high.
There are three types of workouts
that you can include in your program:

Aerobic exercise, strength training, and flexibility training. Aerobic training includes jogging, bicycling, or brisk walking. This is
great for increasing the heart rate, though
you should be careful never to overexert
yourself. Strength training focuses on muscle
strength and function by lifting weights. You
should start at a moderate level, because
movements that require all your strength can
cause injury to the bones and muscle.
Flexibility training involves stretching,
which will expand the motion range of your
joints.
Whether you choose to enlist the
help of a personal trainer or choose to be
your own personal trainer, a good exercise
program will help you to look and feel your
best!

nee pain and knee arthritis are
very common problems to many
men and women, particularly as
they age. The baby boomer generation is
now in their 50’s and 60’s, and total knee
replacement is becoming much more
common. Nearly two thirds of the more
than 400,000 annual knee replacements in
the United States are performed on
women, and the numbers continue to
increase each year. According to a recent
Wall Street Journal article, “Medicine is
not gender neutral. The differences
between men and women are turning out
to be significant in the diagnosis and the
treatment in everything from heart disease

to depression.” In fact, 10 years ago the
government and the medical community
changed the rules for research to include
gender-inclusive studies. The results are
changed in the way medicine, including
orthopaedics, is being practiced.
Men and women are different. In the
case of knee replacement surgery, it is all
about the shape. Replacement knees have
long been available in many sizes, but
this does not always resolve anatomic
differences typically seen in female
patients. The main difference in the
women’s specific knee is the width of the
implant. Women have a slightly narrower

knee for their size and, therefore the women’s specific knee has
taken this into consideration. The knee cap tracks somewhat differently in women. So, the new design specifically for women can
accommodate this with a different angle for the knee cap tracking
mechanism allowing a more natural bend to the knee.In addition,
these are thinner, particularly in the portion of the knee that lies
under the knee cap, more accurately simulating a women’s anatomy.
This new innovation combined with the newer minimally invasive
approach, which results in a much smaller scar, can provide a
quicker return to normal activities making it an ideal joint replacement for women.
physical therapy, injections, or minimally invasive arthroscopic
surgery. However, when the pain and loss of function reaches a
point where the patient is limping, walking is severely impaired and
the pain both day and night becomes unbearable. At this point,
knee replacement may be the best option. Common conditions
which may lead to knee replacement include deterioration of the
knee joint, osteoarthritis, inflammatory arthritis (such as rheumatoid arthritis), moderate to severe bow-legged knee deformity, or to
correct problems caused by previously failed surgery or fractures.
Patients should ask their doctor if they are candidates for one of the
new women’s-specific designed total knees. n

Some painful knee conditions may be treated with medication,
