Or give to Ms. Sandy Novak (Athletic Director) (1) Cougar Foundation Scholarship Application Information Sheet The Cougar Foundation Scholarship will provide two, non-renewable, $1200 scholarships (one male and one female Sahuaro senior). I. a. b. c. d. e. II. Qualification/Eligibility: Students must: Be US Citizen or permanent resident of the United States. Have a seven semester, non-weighted cumulative GPA of: 2.5 – 3.4 Be a high school senior who will enroll at an Arizona accredited two- or four-year college, university or institution of higher learning full-time during the 2012-2013 school year. Have demonstrated leadership abilities through participating in community service, extracurricular or other service-based activities. Have strong motivation to continue their education. Required Documentation Application- Each candidate must submit a complete and timely application. An application will be considered incomplete and not processed if all instructions have not been followed or the material presented is insufficient to permit adequate review. Do not send supplemental or corrected material after the deadline date. Applications and related documents should be neat and easy to read. Materials should be impeccable in terms of appearance and legibility. Submit your application to the Cougar Foundation no later than April 15, 2014. The completed application must include: cover sheet, application and transcript, be accompanied by two letters of recommendation, and essay as well as proof of acceptance at a two or four-year institution of higher learning. The completed application packet should be submitted by mail or to the on-site Sahuaro Cougar Foundation representative, Ms. Sandy Novak, or placed in the Cougar Foundation mailbox found in Sahuaro High School’s teachers’ mailbox room. a. Official Transcript – An unopened, official copy of the applicant’s high school seven-semester, non-weighted cumulative transcript provided by the high school registrar must be supplied in the application packet. Submission of an application for this scholarship constitutes authorization for the Cougar Scholarship Selection Committee to review your academic record. b. Recommendations – Each packet must include two recommendations. One of the recommendations must be from a faculty member who has taught the student in the classroom. Each recommendation must include the reference’s name, address, telephone number, and information regarding how long and in what capacity he or she has known the applicant and a narrative describing the writer’s perception of the ability of the student to succeed in their posthigh school endeavor as well as insight regarding the candidate’s integrity. c. Essay – Each candidate must provide a 500-1000 word essay focusing on “What an Education at Sahuaro High School Means to Me”. The candidate should provide insight into any adversity they may currently have or have had in their life. Essay technical requirements: double space, 12 point font size, having a one (1)-inch margin on all sides.
