Specimen Collection & Rejection Criteria : Completing a Requisition Completing a Requisition The communication of accurate test requests, demographics and additional information required for a specimen to be processed is a critical first step in meeting the needs of the client and patient. To facilitate accuracy in the communication of information, requests for testing will routinely be documented by the physician/client or their representative on a standardized requisition created by Gamma-Dynacare. It is the responsibility of the physician/client to ensure that all demographics, test requests and additional information are accurately provided to the laboratory. Refer to the section on Labeling a Specimen. February 2014 2. Specimen Collection & Rejection Criteria : Labeling a Specimen Labeling a Specimen All specimens received must possess sufficient identification to ensure that the specimens received are traceable to the requisition accompanying the specimens. As a minimum all specimens must be labeled with the following: Patient full last name Patient full first name and One other identifier – either the date of birth (YYYY/MM/DD), internal#, address, phone number etc. Where required specimens and/or requisitions should also be labeled with: Date/time of collection Anatomical sites (where appropriate for swabs, etc.) Time of collection and time placed in fixative Time of draw for multiple specimens (ie Glucose Tolerance Test) NOTE: Specimens received with missing or incomplete information may be subject to rejection. February 2014 2. Specimen Collection & Rejection Criteria : Unlabeled Specimens Unlabeled Specimens Unlabeled specimens will not routinely be accepted for processing. Exceptions will be made to this rule only in situations where the specimen in question is impossible or difficult to retrieve in a timely manner. The following specimens are considered impossible/difficult to retrieve: Histology Biopsy specimens Cytology Pap smears Synovial fluids Blood Cultures Renal Calculi IUD’s (Intrauterine Device) or other foreign objects for culture Electrocardiograms Baby Cord Blood samples February 2014 3. Results Reporting Criteria: Alert & Critical Results Alert & Critical Results Gamma-Dynacare has defined the following 2 categories of phoneable results. These limits have been defined specifically for the community laboratory environment and are approved by the Medical Director for use. LEVEL I – (Critical) – A result that will cause the patient to suffer a lifethreatening event if not communicated and treated immediately. These results show marked deviation from reference intervals suggesting that, if unexpected, a patient’s life is in danger and prompt medical action may be required. These must be called as soon as results are available 24 hours per day. In addition to phoning, these results will be faxed. LEVEL II – (Phoneable/Alert) – A result that deviates significantly from the reference interval and if unexpected, suggests the need to re-evaluate the clinical situation prior to the arrival of a report by routine channels. These results are phoned and faxed between 07:00 and 20:00 hours. February 2014 4. Specimen Collection: Order of Blood Collections Draws Order of the Draw To prevent contamination of tubes with additives from other tubes it is important to draw the tubes in a SPECIFIC order called “the order of the draw”. For example, if the additive in the purple stopper tube contaminates the green stopper tube this would cause falsely decreased calcium and increased potassium. The sequence of collection of evacuated tubes in a multi-draw should be in this order: February 2014 4. Specimen Collection: Centrifugation Centrifugation of Specimens The centrifuge is a device used to accelerate gravitational separation of substances differing significantly in their masses. The quality of the separation obtained is directly dependent upon the type of centrifuge used and the manner in which the centrifuge is used. Ensure that tubes of equal volume are placed opposite each other in the buckets. Extra tubes filled to different volumes with distilled water may be used to balance tubes if required. NOTE: Never attempt to balance by adding weights or other objects to the bottom of the tube. February 2014 5 . Refrigerate and protect from light during collection. Please discard the first morning specimen. Continue to collect all specimens for the rest of the 24 hours.
