OH-2 06/14 120,001lbs – 132,000: 7 axle vehicle with at least 65’ of overall spacing and 51’ of innerbridge spacing all weights from routine apply. 132,001 and up: need at least three components (truck, trailer, booster or truck, jeep, trailer) with 12’6’’ of spacing between the groups and 4’1’’ of spacing between the axles within the group. OHIO SPECIAL HAULING PERMITS SUPERLOAD VEHICLE CONFIGURATION This covers both routine and superload applications. All Loads must first meet the minimum spacing requirement for overweight which is 51’ of overall spacing and 36’ of inner-bridge. After meeting this requirement is met, the spacings or number of components for overweight will not be considered, instead axle weights and axle group weights along with a passed analysis (via the SC&RA Oversize/Overweight Permit Manual Superload program) will determine if a permit will be issued. If an application fails a bridge analysis or exceeds the stated group weights, either more axles or more components must be added in order to appropriately spread the weight over the vehicle. Once a vehicle exceeds 120,000 lbs. GVW, it is still considered a superload, but the minimum spacing of 65’ overall spacing and 51’ of spacing for the inner-bridge will not be required. Also, three components will no longer be required when a vehicle exceeds 132,000 lbs. provided the maximum axle or axle group weights are not exceeded and a passed analysis is obtained. The requirement for equalized weight, (2,000lbs. between axles within a group) and all other rules still apply. The Department recognizes that the use of lift axles is necessary for overweight superload weight distribution and that these axles must be lifted during some turning maneuvers. When such axles are in use and listed as load bearing axles on an overweight superload SHP, they shall remain in the fully lowered position except during turning maneuvers where there would be excessive tire and pavement scuffing or control of the vehicle would be hindered. Such axles shall be part of an air ride suspension group designed to equalize the load over all axles in the group, including the retractable axles. October 27, 2011 WIDTH: HEIGHT: LENGTH: 14' 14'6" depends on routes PERMIT RESTRICTIONS General - Permits are valid for five days. Travel is permitted one-half hour before sunrise to one-half hour after sunset, travel until 3 p.m. on Saturday and Sunday. No travel on holidays. Permits may be extended for a justified reason (weather, tractor breakdown), but only one revision is allowed. The load description cannot be changed under an extension. Permits may be issued to haul a dozer with the blade attached, but a blade protector is required if the vehicle exceeds 12' in width (but not over 14' in width). When a blade protector is in use, the dozer is to be loaded with the blade to the rear of the trailer. Blades 15' in width and greater must be removed and hauled separately. Every vehicle operating under a Special Hauling Permit shall, when traveling on freeways, expressways, multi-lane undivided highways, remain in the extreme right-hand lane of said highway except as necessary to maintain continuous through movement, to make left turns or exits or to pass other vehicles. Overwidth vehicles shall not pass other vehicles traveling in the same direction. Any load with an overall height in excess of 14'10" shall be required to coordinate the move with the owners of all overhead signs, signals, utilities, etc., which may obstruct safe, clear movement. Divisible load permits are not available. OH-3 06/14 SC&RA Oversize/Overweight Permit Manual Insurance — The current minimum property damage (automobile) liability insurance amount to be endorsed or surety bond amount is $500,000.00 and may be increased in cases where an increased risk is determined. In cases of self-propelled equipment, general liability may be substituted for the automobile liability. This must be filed on forms prescribed and furnished by the Ohio Department of Transportation, Special Hauling Permit Section. Please note: Both the OS-32 form and the bond form must be completed and signed by an authorized representative of the insurance company and sent to us directly from them. The forms available on our website are fillable pdf documents. A blank copy of the form(s) can be saved. Once the OS-32 form has been completed and signed by an appropriate signatory, it may be mailed, faxed to (614) 728-4098 or emailed to [email protected] Weekend/Night Travel – The Ohio Special Hauling Permit Limitation / Provision Form OS-1A states that permitted movements may not occur during certain hours of the day. These restrictions are summarized as follows: 1. OVERWEIGHT ONLY permitted vehicle/loads do NOT have any travel hour restrictions 2. OVER DIMENSIONAL permitted vehicle/loads (Over - length or width or height) may NOT travel during listed holidays. For the list, see the Form OS-1A or click on Main Page for Holiday Restrictions. This provision does NOT apply to FARM Equipment permits. 3. OVER WIDTH permitted vehicle/loads greater than 12 feet wide may NOT travel during listed morning and afternoon "rush hours" near listed cities. For the list, see the Form OS-1A. This provision does NOT apply to FARM Equipment permits. 4. OVER WIDTH permitted vehicle/loads greater than 12 feet wide may NOT travel during listed weekend hours. For the list, see the Form OS-1A. This provision does NOT apply to FARM Equipment permits. 5. OVER DIMENSIONAL permitted vehicle/loads (Over - length or width or height) may ONLY travel during daylight hours (defined as 1/2 hour before sunrise and 1/2 hour after sunset). Holiday – 2014 HOLIDAY RESTRICTIONS Due to increased traffic on our highways during holiday periods, the movement of oversize vehicles/loads is limited. This is in accordance with Ohio Administrative Code Section 5501:-09(A). Overweight only vehicles/loads may move on holidays/holiday weekends if they are traveling with a valid Special Hauling Permit. Overdimension and/or overdimension/overweight vehicles/loads may not move on the holidays/holiday weekends listed below. DAY -JANUARY 20, 2014 PRESIDENTS’ DAY –FEBRUARY 17, 2014 COLUMBUS DAY (Observed)-OCTOBER 13, 2014 VETERANS DAY -NOVEMBER 11, 2014 OH-4 03/14 SC&RA Oversize/Overweight Permit Manual Inclement Weather - None Noted Permit Information Required — Tractor and trailer information; description of load (make and model); net weight; gross weight; axle spacings; axle weights; number and size of tires; load dimensions; overall dimensions; origin and destination; suggested routes to be traveled; effective dates. PERMIT TYPES Active or Single Trip – One way movement Continuing – Issued for repeated movements of the same vehicle and load over same routes; issued for ninety days or one year. Three Steel Coil permits - Allowed for gross vehicle weight up to and including 120,000 pounds for a single trip permit and for a continuing ( formerly called "90 Day" ) permit. Marina Permit.- A Marina permit has been formalized. The permit will be issued for an unlimited number of movements of boats between the marina and points of storage, repair, or launch within a 10 mile radius. Permit will not exceed legal weight, nor exceed 14 feet overall width, or exceed 65 feet overall length. Movement is not authorized on fully controlled or limited access highways. Construction Equipment – Movement of construction equipment, up to 12' in width and legal weight, to and from job site (one year). FEES Single Trip – OS only $65 one way $100 and return, OS/OW $135 one way $200 and return, Steel Coil $65 one way, Multi-State OS only $65 one way, Multi-State OS/OW $135 one way, and Emergency $250 one way $365 and return. Continuing (90day) – OS only $250 one way $375 and return, OS/OW $500 one way $750 and return, Steel Coil $125 one way, Michigan Legal $125 one way $125 and return. Continuing Annual (365 day) – OS only $970 one way $1,170 and return, OS/OW $1,970 one way $2,970 and return, Steel Coil $470 one way, Michigan Legal $470 one way $470 and return. Construction Equipment – Fee of $100. ESCORTS AND SIGNS Labeling — Signs are required on the front and rear of all oversize vehicle. The sign must have a yellow background with black capital letters 10" high, and have dimensions of at least 7' x 1.5'. Letters should be at least 10" high with a 1.41" brush stroke. Sign must read "OVERSIZE LOAD" Flags must be displayed at all four corners of an overwidth vehicle, and must be red or orange and 18" square. Escort Vehicles and Personnel — 1. Escort vehicles, when required by a Special Hauling Permit to accompany an overdimensional or overweight vehicle or vehicle/ load, shall be required to display a warning sign, yellow with black letters, reading “OVERSIZE LOAD.” The sign shall be 5' long by 12" high with 8" high letters. Escort vehicles shall also be required to maintain radio communication with the operator of the permit vehicle and shall also be required to be equipped with a roof mounted amber flashing or rotating light(s) visible at a minimum distance of 1,000 feet in normal daylight conditions. Driver of the escort vehicle is to act as a flagman when needed. 2. One rear escort vehicle shall be required for the transportation of any vehicle/load with an overall length in excess of 90'. 3. One lead (rear on multiple lane highways) escort vehicle shall be required for the transportation of any vehicle/load with an overall width in excess of 13'. One lead escort vehicle equipped with a height sensing device shall be required for the transportation of any vehicle/load with an overall height in excess of 14'6". 5. One lead and one rear escort shall be required on any vehicle or vehicle/load with an overall width in excess of 14'6". 6. One lead and one rear escort shall be required on any vehicle or vehicle/load with an overall height in excess of 14'10". 7. If more than one of the conditions set forth in numbers 2. through 4. above are met (for example, a load exceeding 13' in width and 90' in length), two escorts (one lead and one rear) shall be required. 8. Lead escort vehicle, when required, is to precede 500' in advance of permit vehicle/load. Rear escort vehicle, when required, is to follow 500' back of permit vehicle/load. Escort requirements are described in the Hauling Permits Operational Guide (Section XII pages 11 – 12) and also on the OS-1A Limitation and Provision sheet. When escorts are required, the permittee is responsible for making arrangements for all escorts, including police escorts. An escort vehicle driver must be at least 18 years old and have a valid driver’s license issued by the state in which the escort vehicle is registered. If your permit calls for two private escorts to accompany the load during movement. If you wish, you may travel with one private escort and one Ohio State Highway Patrol (OSHP) escort if the OSHP has a person available and are willing to escort the load. Certification – Pilot Car Escort Certification is not currently required. FINES Overdimension Only: Range from $25 to $100 per violation Overweight Fines: 0-2,000 lbs. $ 80 2,001-5,000 lbs 5,001-10,000 lbs. over 10,000 lbs. 100, plus $1 per 100 lbs. 130, plus $2 per 100 lbs. 160, plus $3 per 100 lbs. Minimum fine is $100. Falsification of weights on bills of lading, dock receipts, etc-$5,000, plus 30 days in jail. Operators may be fined for either over-axle or over-gross, and may be permitted to shift up to 1,000 lbs to avoid an over-axle citation. OH-6 03/14 SC&RA Oversize/Overweight Permit Manual Fees for permits on the Ohio Turnpike: $4 per permit, plus: $.02 per mile for each foot or fraction overwidth; $.01 per mile for each foot or fraction overlength; $.02 per mile for each foot or fraction overheight. Other requirements: Doubles combination vehicle operators must obtain special permission from the Turnpike Commission to operate on the turnpike, and may have minimum dimensions of 90'–106', and a maximum gross weight of 127,000 lbs. GVW. Slow-Moving Vehicles or Equipment Crossing Railroad Tracks. No person shall operate or move any crawler-type tractor, steam shovel, derrick, roller or any equipment or structure having a normal operating speed of six or less miles per hour or a vertical body or load clearance of less than nine inches above the level surface of a roadway, upon or across any tracks with a railroad grade crossing without first complying with divisions (A) and (B) of this section. (A) Before making any such crossing, the person operating or moving any such vehicle or equipment shall first stop the same, and while stopped he shall listen and look in both directions along such track for any approaching train and for signals indicating the approach of a train, and shall proceed only upon exercising due care. (B) No such crossing shall be made when warning is given by automatic signal or crossing gates or a flagman or otherwise of the immediate approach of a railroad train or car. If the normal sustained speed such vehicle, equipment, or structure is not more than three miles per hour, the person owning, operating or moving the same shall also give notice of such intended crossing to a station agent or superintendent of the railroad, and a reasonable time shall be given to such railroad to provide proper protection for such crossing. Where such vehicles or equipment are being used in constructing or repairing a section of highway lying on both sides of a railroad grade crossing, and in such construction or repair it is necessary to repeatedly move such vehicles or equipment over such crossing, one daily notice specifying when such work will start and stating the hours during which it will be prosecuted is sufficient. Permit requests can be made via the department's web site at https://ohpass.dot.state.oh.us/ohpass/login.asp, and additional information is also available online at http://www.dot.state.oh.us/Divisions/Operations/Maintenance/Permits/Pages/default.aspx. Obtain a user login name and password from ODOT Special Hauling Section by completing an OS – 7 Form and have your insurance company provide ODOT with a completed OS – 32 Form. SPECIAL NOTES Recent Changes – The Ohio Permit Office has moved to 1980 West Broad Street. The telephone and fax numbers will remain the same.
