Student Awards & Financial Aid Dear OSAP Applicant: The Ministry of Training, Colleges & Universities has placed your OSAP application on hold due to a review of your academic history. One of the criteria of the Ontario Student Assistance Program is that recipients must be progressing satisfactorily in their studies. Once students are placed on OSAP Probation, their academic status must be monitored. One or more of the following situations may have occurred: At some point you withdrew from your full‐time studies while receiving OSAP assistance. You did not pass at least 60% of a full course load (40% for students with a Permanent Disability) in a term for which you received OSAP assistance. Voluntary withdrawals, audit status and failing grades constitute unsuccessful academic progress. You have taken more than one additional term to complete your program requirements. You have received OSAP assistance from more than one school or have been registered in a particular year of your program for more than 1 academic year (e.g. in year 3 for two consecutive years). You were restricted from OSAP previously due to unsuccessful academic progress and have been restricted from receiving OSAP assistance for a period of 12 months. SUPPORTING DOCUMENTATION REQUIRED for OSAP PROBATION NOTIFICATIONS: You have received an OSAP PROBATION (1st INSTANCE) email notification from the Student Awards & Financial Aid Office at the University of Windsor and/or an OSAP PROBATION (1st INSTANCE) letter from the Ministry of Training, Colleges and Universities and you DID NOT experience any extenuating circumstances that might have impacted your ability to successfully complete the term: o You must provide a letter of explanation outlining what steps/strategies that you will take to improve your academic progress and a description of your academic/career goals. Please use the attached Academic Progression Review Cover Sheet. If you DID experience extenuating medical, personal or family circumstances that did impact your ability to successfully complete the term resulting in the OSAP Probation (1st Instance Notification) being sent to you and you would like to appeal to request to have the OSAP Probation status removed from your OSAP application, you must provide the following: 1|Page o A letter explaining in detail, the nature of the extenuating circumstances (medical, personal or family) that prevented you from successfully completing the term. o Supporting documentation to corroborate or verify the information that you are presenting. For example, in the case of illness, a letter from your physician confirming the nature of the illness and the time period that you were deemed to be unfit to attend classes. In the case of extenuating personal circumstances, a letter from a counselor, teacher or other professional third party confirming the nature your difficulties, the period of time affected, and how your difficulties impacted your ability to maintain your studies to the level required. Please attach your letter and supporting documentation to the Academic Progression Review Cover Sheet. Your OSAP application, current year and subsequent years will remain on hold until your 1st instance of identified academic progression issues has been addressed, either by your letter of explanation with respect to your strategies for academic improvements or by your letter of explanation regarding your extenuating circumstances experienced with your supporting documentation. SUPPORTING DOCUMENTATION REQUIRED for OSAP RESTRICTION NOTIFICATIONS: You have received an OSAP RESTRICTION (2nd or 3rd INSTANCE) email notification from the Student Awards & Financial Aid Office at the University of Windsor and/or an OSAP RESTRICTION Notice (2nd or 3rd INSTANCE) letter from the Ministry of Training, Colleges and Universities AND you experienced extenuating medical, personal or family circumstances that did impact your ability to successfully complete the term resulting in the OSAP Restriction (2nd or 3rd Instance Notification) being sent to you and you would like to appeal this restriction, you must provide the following: o A letter explaining in detail, the nature of the extenuating circumstances (medical, personal or family) that prevented you from successfully completing the term. o Supporting documentation to corroborate or verify the information that you are presenting. Please attach your letter and supporting documentation to the Academic Progression Review Cover Sheet. IMPORTANT NOTE if APPEALING your OSAP Probation/Restriction: Submissions that are not accompanied by supporting documentation to support the statements made in your letter of explanation will not be accepted. Your letters of explanation must be signed and dated and your Student ID# must be clearly noted on your letter. 2|Page SUPPORTING DOCUMENTATION REQUIRED for OSAP REINSTATEMENT REQUESTS: You have been out of full‐time study for a period of 12 consecutive months (1 year) or more and you have satisfied the requirements of your restriction period. Please submit the following documentation to our office for review: o A completed Academic Progression Review form (attached); o A letter detailing your situation and the reason for the withdrawal or failure. Your letter should include the following information: Your current year of study; How many terms you have accessed OSAP assistance; How many more terms you will require OSAP assistance; The amount of your total student loan debt; The reasons you have not successfully completed previous OSAP‐funded term(s); Your academic plan for your future OSAP funded studies. IMPORTANT NOTE: After you have been placed on OSAP probation, you will be on probation for a period of 1 year. If after one year, you have successfully completed 60% of a full course load averaged over the course of the academic year, the Student Awards & Financial Aid Office will request to have the OSAP probation flag removed from your file. If however, this is not done systematically, please use the Academic Progress Review form to request to be removed from the conditions of OSAP probation as you have met the requirements of your probation term. FOR ALL ACADEMIC PROGRESSION REVIEW TYPES: Please allow a minimum of 4 weeks for your review to be completed and submissions should be received at least 6 weeks prior the start of your study period. Your submission should be typewritten. Ensure all documentation is signed, dated and that your Student Number is clearly noted on your submission. Please be sure that your supporting documentation corresponds to the extenuating circumstances that you have outlined in your letter.
