Subject: oneChart System Upgrade *October 22, 2014 Changes* Facilities Impacted – All Facilities (Acute and Ambulatory) – All Users Effective October 22, 2014, the oneChart (Cerner) system will be upgraded to a new code level, 2012.01.29 on the morning of Wednesday, October 22, 2014. More specific information will be provided in a flyer, which will be distributed prior to the implementation date. 2. Subject: Order Changes and Alerts *October 21, 2014 Changes* Facilities Impacted – All Adult Facilities Formulary Antibiotics Plan, Sepsis 6 Hour Bundle Plan, and Severe Sepsis 6 Hour Bundle Plan – Physicians, Physician Related Roles, and Nursing The Antibiotic Formulary Plan will be renamed Commonly Used Antibiotics Plan. The Commonly Used Antibiotics Plan will be added to the Sepsis 6 Hour Bundle Plan and the Severe Sepsis 6 Hour Bundle Plan. Facilities Impacted – All Adult Facilities TTE Adult Echo W/Contrast if Necessary and Transesophageal Echocardiogram Adult Orders – Physicians, Physician Related Roles, and Nursing The TTE Adult Echo W/Contrast if Necessary (Transthoracic Echo Adult) order will be renamed TTE Echo W/Contrst or 3D if needed and the Transesophageal Echocardiogram Adult order will be renamed Transesophageal W/3D if needed in the oneChart system and the Cardiology PACS system. *October 28, 2014 Changes* Facilities Impacted – All Facilities Parent/Child Orders – Physicians, Physician Related Roles, and Nursing When orders are placed to be completed on a recurring basis, the orders will display with a parent order and child orders. To view orders with multiple instances, click the Displayed blue hyperlink in the Orders tab, select the Show individual instances for continuing orders option in the Miscellaneous pane, and click Apply in the Advanced Filters window. After the Show individual instances for continuing orders option is selected, a delta will precede the parent order(s). Click the delta to view the recurring orders or individual instances associated with the parent order. Canceling/discontinuing the parent order of most orders will cancel/discontinue the individual order instances to prevent any active child orders from being completed. It is important to review the child orders to ensure the individual instances have been canceled/discontinued. Lab orders, which have been dispatched, will usually remain in an ordered status. Nursing must call the Laboratory department to cancel/discontinue dispatched lab orders. When necessary, the individual child orders may be completed, canceled/discontinued, or voided. Nurses will need to address the related Activity List tasks on both the Scheduled and Nurse Collect tabs. Facilities Impacted – All Adult Facilities ANES Sleep Apnea Plan – Physicians, Physician Related Roles, and Nursing The Oxygen Saturation Continuous Monitoring RT T;N, Routine order will replace the O2 Sat Continuous Monitoring NSG order in the ANES Sleep Apnea Plan. Facilities Impacted – All Adult Facilities NM Myocardial Perf SPECT Mult Study ( Cardiac Stress Test- Non specified) Order – Physicians, Physician Related Roles, and Nursing A prebuilt order sentence with the choice to order for Today will be added to the NM Myocardial Perf SPECT Mult Study (Cardiac Stress Test – Non specified) order. This order sentence option will be added to the NM Myocardial Perf SPECT Mult Study (Cardiac Stress Test – Non specified) order in the Cardiac Stress Test Orders Careset. Facilities Impacted – All Facilities Mechanically Ventilated Patient Orders Careset – Physicians, Physician Related Roles, and Nursing The new order, Reposition ETT QD, will be added to the Mechanically Ventilated Patient Orders Careset. When the Reposition ETT QD order is placed, a task, Collaborate with Respiratory Therapy to ensure ETT is repositioned, will display on the nurse’s Activity List. Facilities Impacted – All Adult Facilities Announcement Neuro Stroke Labs Plan – Physicians, Physician Related Roles, and Nursing The new PowerPlan, Neuro Stroke Labs Plan, will be available for use. Facilities Impacted – All Adult Facilities PEG Pre-insertion Plan – Physicians, Physician Related Roles, and Nursing The new PowerPlan, PEG Pre-insertion Plan, will be available for use. Facilities Impacted – All Adult Facilities RAD Chemoembolization Pre Proc Plan – Physicians, Physician Related Roles, and Nursing Modifications will be made to the start date and time on multiple orders, and the collection priority will be changed from Routine to STAT on all laboratory orders in the RAD Chemoembolization Pre Proc Phase of the RAD Chemoembolization Pre Proc Plan. Facilities Impacted – All Adult Facilities Radiology MRI Contrast Plan – Physicians, Physician Related Roles, and Nursing The order, Magevist 15 mL, IV Push, N/A, Routine, Comments: RAD BILL ONLY, will be added to the Radiology MRI Contrast Plan. Facilities Impacted – All Adult Facilities except Fayette LEB ED Febrile Neonate Plan and PED ED Febrile Neonate Orders Careset – ED Physicians, ED Physician Related Roles, and ED Nurses Multiple revisions will be made to orders in the LEB ED Febrile Neonate Plan and in the PED ED Febrile Neonate Orders Careset. Facilities Impacted – Le Bonheur Le Bonheur NICU Admission PowerPlans – Physicians, Physician Related Roles, and Nursing The Consult Case Management T;N, Reason for Consult: Discharge Planning order will be preselected in all LEB NICU admission PowerPlans. 3. Subject: Documentation Changes *October 21, 2014 Changes* Facilities Impacted – All Facilities Iview Visitation/Education Band, Medication Education Section, Medication Education Topics Field – Nursing, Pharmacy, Dietary, Rehabilitation, and Respiratory Therapy A free-text field will replace the list of options in the Medication education topics field in the Medication Education section of the Visitation/Education band in Iview. Facilities Impacted – All Facilities PNED ED Provider Note Pain Assessment – ED Physicians, ED Physician Related Roles, and ED Nurses The Pain Assessment sentence will be added to the Physical Examination paragraph of the ED Provider Note templates in PNED. The Pain Assessment sentence will contain the term, Pain Level: include nursing pain assessment, which will add the pain scale from the ED nurse’s documentation in the ED provider’s note. In addition, the Pain Assessment sentence will contain the Pain management: Refuses initial offer of pain medication term to assist ED providers in documenting the patient’s refusal of initial pain medication in order to meet CMS requirements. *October 28, 2014 Changes* Facilities Impacted – All Adult Facilities ED Triage Form ED Sepsis Screening Tool Section – ED Physicians, ED Physician Related Roles, and ED Nurses When the Pt > or = 18 and not at Le Bonheur option is selected in the Sepsis screen field of the ED Triage Form, the ED Sepsis Screening Tool section will display for further documentation. When the Positive sepsis screen option is selected in the Sepsis screening result field, the Severe sepsis criteria field becomes required in the ED Sepsis Screening Tool. In addition, a rule will automatically place the following lab orders: CBC, CMP, and Lactic Acid Level. The system will place the Blood Culture q15 min x 2 order when the Positive sepsis screen option is selected in the Sepsis screening result field and the Temp < or = 36 or > or = 38 deg C is selected in the SIRS screen field of the ED Sepsis Screening Tool. Facilities Impacted – All Adult Facilities Handoff Form and ED Depart Form – ED Physicians, ED Physician Related Roles, and ED Nurses A new field, Sepsis Alert Criteria met, which will contain Yes and No options, will be added to the Handoff Form and the ED Depart Form.
