Similarly to burning mouth syndrome, neurophysiologic signs indicative of neuropathic dysfunction both at the peripheral and the central levels of the trigeminal system, and deficient dopaminergic inhibition were found, first time highlighting the neuropathic nature of this pain condition. Has led to a modification of the operation technique that is less traumatic to the neurovascular bundle, and decreased the incidence of these iatrogenic neuropathies in our hospital. (IF 1.259) The first publication I am aware of comparing the diagnostic value of several clinical, quantitative sensory and neurophysiologic tests in the diagnosis of sensory nerve injury in a prospective follow-up study design that also utilized the data from intraoperative monitoring as the gold standard of nerve injury. First study highlighting in detail the fact that clinical sensory examination is a poor diagnostic tool for the diagnosis of traumatic peripheral sensory neuropathy, already at the acute stage. (IF 5.836) The first PET study on any clinical pain condition showing an imbalance of the striatal dopamine D1 and D2 receptor mediated pathways that may indicate either a decrease in the synaptic endogenous dopamine content or an increase in D2 receptor density in the putamen of burning mouth patients. Supplies direct evidence for a deficit in the top-down dopaminergic control in clinical pain that has later been shown to be of neuropathic origin. (Number of citations 92) 11. Teerijoki-Oksa T, Jääskeläinen SK*, Forssell K, Virtanen A, Forssell H: Recovery of nerve injury after mandibular sagittal split osteotomy. Diagnostic value of clinical and electrophysiologic tests in the follow-up. Int J Oral Maxillofac Surg 2004;33:134-140. (IF 1.359) Showed clearly the superiority of clinical neurophysiologic recordings and, to a slightly lesser degree, quantitative sensory tests in the late diagnosis (after 6 months) and follow-up of recovery of iatrogenic inferior alveolar nerve injuries. At one year, clinical sensory tests were able to detect only 7% of the still symptomatic nerve injuries whereas the combination of neurophysiologic and psychophysical sensory tests was 100% sensitive. Showed, for the first time, the systematic, dose-dependent epileptogenic effect of deep sevoflurane anaesthesia in healthy subjects with no previous personal history of epilepsy. The results indicate that neurophysiologic EEG recordings are critical in the evaluation of the CNS effects of new anaesthetics. (Number of citations 64) 13. (IF 5.836) The first neurotransmitter PET study on atypical facial pain patients. Showed similar imbalance of the striatal dopamine D1 and D2 receptor mediated pathways as we previously found in burning mouth syndrome. The findings may indicate either a decrease in the synaptic endogenous dopamine content or an increase in D2 receptor density in the putamen. Pain 2009;142:149-158 (IF 5.836). First study ever using E-field navigated precision TMS-device to explore the cortical brain targets that may induce analgesic effects in healthy subjects. For the first time, it could be shown that the right secondary somatosensory cortex might be a more suitable target to induce analgesia than the previously used targets at the primary motor cortex and dorsolateral prefrontal cortex. In addition, the use of signal detection theory based mathematics enabled dissection of the changes in sensory thresholds to those due to changes in sensory discriminative capacity and to those due to altered subjective criterion, i.e. nonsensory aspects of psychophysical test results that may be the most important factor in clinical effects of rTMS. These findings decisively confirm my early hypothesis (formed in 1997) of the role of brain dopamine system in experimental as well as clinical pain.
