In order to receive your refund on a DELSYM® product, you must complete the information above with your name, address,
phone number and e-mail. Only one offer per household or address, and cannot be combined with any other Delsym rebate
offer. Offer is only available to legal residents of the 50 US states and District of Columbia. To receive your refund, please
provide your name and complete address on the form and send with your original cash register receipt dated between
October 1, 2014 and October 31, 2014. Show the purchase of a DELSYM® product with the UPC noted on the receipt. No
other UPC will be honored. No copies of the cash register receipt will be accepted. All refund offers must be postmarked no
later than November 15, 2014. Void where Prohibited. Please allow 8 to 10 weeks to receive your refund in the form of a
check for the exact amount paid for the product, up to $14.
