 This must have signature of the applicant.
CKGS Order Form
 This must have signature of the applicant.
Current US Passport
 Current US Valid Passport with minimum of 180 days validity
 Two blank pages
 Last two observation pages are not considered as part of the
2 blank pages
 Copy of the information page that contains the picture and
basic details of the passport.
Proof of Address
Proof of address to match with present address as per application.
Only one of the following:
 Only State Issued - Driving license

 5 year and 10 year Tourist visas are available only to US citizens under a bilateral arrangement.
 The Govt. of India has directed that, apart from special circumstances, Indian Missions / Posts in
the United States of America will grant Multi entry Tourist Visa for a period of 10 years to
Nationals of United States of America
 All non-US passport holders are only eligible for up to a Six (6) month Tourist visa.
 All the Non US applicants should mention their permanent address in their home country
failing which the application will not be processed.
