TO VOTE IN FLORIDA YOU MUST SHOW A PICTURE AND SIGNATURE ID To vote, other than by provisional ballot, you must show current and valid picture and signature identification. Acceptable forms of ID are: valid Florida driver license, Florida ID issued by DHSMV, U.S. passport, signed debit or credit card, military ID, student ID, retirement center ID, neighborhood association ID or public assistance ID. Expired ID’s are not valid. Any voter whose eligibility is in question may vote a provisional ballot during Early Voting or on Election Day. You may provide written evidence supporting your eligibility to vote to the Supervisor of Elections at 105 E. Monroe Street, Jacksonville, FL 32202 no later than 5 p.m. on the second day following the election. All provisional ballots are reviewed by the canvassing board prior to certification of election results. If it is determined that the voter in question is eligible to vote, his/her ballot will be counted. Any voter who votes a provisional ballot will be given a reference number and the office phone number (630-1414) to call to determine the outcome of his/her eligibility. CHANGE OF PERMANENT ADDRESS, NAME AND SIGNATURE If you are registered to vote in Florida, you may change your residential address or name up through Election Day and still be able to vote in your precinct. Signature updates must be made on a voter application and submitted no later than the start of canvassing absentee ballots which is October 20, 2014. Important Message to Voter Dear Duval County Voter: The 2014 Duval Primary election was truly a civics lesson. We conducted two recounts – in a statehouse race that came down to TWO votes and a school board race for second place and the run-off came down to FOUR votes. Don’t let anyone say that an individual vote doesn’t count. The candidates in these two races know differently! In order to minimize the cost to the taxpayers, we are mailing sample ballots to households and not to individual voters. We ask you to share this sample with all the registered voters in your household. The General ballot is two sided. The constitutional amendments appear on side two. The municipal elections of Jacksonville Beach and Neptune Beach also appear on side two. Please vote both sides of your ballot. Due to the length of the ballot, we anticipate some waiting at the polls. We encourage voters to vote early or by absentee. VOTER BILL OF RIGHTS Each registered voter in the State of Florida has a right to: 1.Vote and have his or her vote accurately counted. 2.Cast a vote if he or she is in line at the official closing of the polls in that county. 3.Ask for and receive assistance in voting. 4.Receive up to two replacement ballots if he or she makes a mistake prior to the ballot being cast. 5.An explanation if his or her registration or identity is in question. 6.If his or her registration or identity is in question, cast a provisional ballot. 7.Written instructions to use when voting, and, upon request, oral instructions in voting from elections officers. 8.Vote free from coercion or intimidation by elections officers or any other person. 9.Vote on a voting system that is in working condition and that will allow votes to be accurately cast. The deadline was October 6th, 2014 for the November 4, 2014 General Election. The deadline for updating your signature for the purpose of voting by mail and provisional voting is prior to the start of canvassing which falls on October 20, 2014. VOTER RESPONSIBLITIES Each registered voter in this state should: 1. Familiarize himself or herself with the candidates and issues. 2. Maintain with the office of the Supervisor of Elections a current address. 3. Know the location of his or her polling place and its hours of operation. 4. Bring proper identification to the polling station. 5. Familiarize himself or herself with the operation of the voting equipment in his or her precinct. 6. Treat precinct workers with courtesy. 7. Respect the privacy of other voters. 8. Report any problems or violations of election laws to the Supervisor of Elections. 9. Ask questions, if needed. 10.Make sure that his or her completed ballot is correct before leaving the polling station.
