TERMS & CONDITIONS / SUBSTITUTION / CANCELLATION
• Registration forms to be sent by hand or email only.
• The registration form serves as contract between the participant and SPAD.
Thus only fully completed registration forms will be accepted.
• Registration fees do not include flights or accommodation.
As soon as your registration form is received, you will receive a confirmation
email.
• If payment is to be effected by bank transfer, please ensure that all costs for
the transfer are borne by the payee and payment is effected NETT. Bank
charges have to be borne by the participant.
• If making payment by cheque, please state name of company, contact person
and contact phone number on the back of the cheque. Please state
‘LPTSymposium2014’ on the bank-in slip. Then email the copy of the bank-in
slip to [email protected].
• Please send proof of payment and reference number to [email protected]
after payment has been made.
•Please note that payment needs to be made by 10 NOVEMBER 2014 and that entrance
to the dinner and/ or conference is assured only upon receipt of full registration fees.
• Registered participants whose payment is not settled will not be allowed to
participate in the conference.
• The organisers reserve the right to stop any registered delegate from taking
part in the event if no proof of payment can be presented.
• If a delegate cannot attend the event, we will accept a substitute colleague.
• All requests for substitutions must be submitted BEFORE 14 NOVEMBER 2014
& will only be accepted by e-mail addressed to [email protected]
indicating the name of the cancelled delegate and the substitute.
• The substitute must be from the same organisation.
• For cancellations, a refund minus 10% service charge will be sent to the said
delegates if cancellation is received in WRITING by 10 November 2014. The
refund will be made, payable within 14 days.
• No refund will be made for cancellation received after 10 November 2014.
• No-shows will not qualify for a refund of fees.
• These rules are also applicable for group delegates.
• SPAD reserves the right to make amendments to the programme or any related
activities at its discretion.
