JOB DESCRIPTION FAMILY SUPPORT WORKER (12 Month Pilot Scheme) in the first instance Hafal’s Internal Job Title for this post is Recovery Practitioner (2) Hours: 15 per week Salary: £16,569 – 18,599 (pro rata) Holidays: 25 days per year (plus 8 public holidays) Period of Notice: 1 week in the first 6 months; 4 weeks thereafter Probationary Period: 6 Months but can be extended if needed Based at: Flintshire Substance Misuse Team, Shotton Responsible to: Service Manager Main Relationships Internal: Hafal staff, Hafal members Main Relationships External: Social Services, Substance Misuse Team, CMHT, Clients and Carers Job Purpose: To improve the lives of people with a mental illness and/or a substance misuse problem and their families/carers by providing one-to-one and group support to families where a member of the family is affected by both a mental illness and/or a substance misuse problem, and to equip them with skills, knowledge and coping strategies to ensure their own well-being. Hafal’s services are guided by our innovative Recovery Programme (described in detail in “Recovery - the way ahead for people with severe mental illness”), the key components of which are:- The Programme addresses needs holistically, methodically supporting clients in relation to Physical health Medication for mental illness Other treatment and therapy for mental illness Employment Training & Education Money Accommodation Parenting and caring relationships Social life Main Duties and Responsibilities: Receiving and dealing with referrals Providing information and advice Providing support Liaising and working jointly with other agencies Signposting/referring on to other specialist/appropriate services Providing services on a one-to-one casework basis Providing services on a group basis Providing services on a peripatetic and on-site basis Attending training courses and other meetings as required Maintaining records and statistics required to monitor and evaluate the effectiveness of the service Carrying out all reasonable tasks commensurate with the post as directed by line manager Implementing the Hafal Equal Opportunities Policy and all other Hafal policies and procedures Working in accordance with Hafal’s Recovery Programme 1 RECOVERY PRACTITIONER (2) PERSON SPECIFICATION Criteria Desirable in Italics. Qualification • There are no specific qualifications but there must be evidence of commitment to personal and career development relevant to the post. • NVQ Competencies in Care Level 3 would provide some evidence of competence for the post. Must have experience of providing care and support services to people with a substance misuse problem or their carers. OR Must have experience of providing care and support services to people with a substance misuse problem and a mental illness and/or their carers. OR Must have experience of providing care and support services within a social care setting • Experience • • Knowledge • • • Ability Must have a sound understanding of the social care needs of people with a substance misuse problem. Must have sufficient understanding of the association between substance misuse and mental illness.
