Team fee: 11 v 11: $450+ HST = $508.50
Team Fee: 9 v 9: $385 +HST = $435.05
Team fee: 7 v 7: $ 275+ HST = $310.75

TEAM MANAGER CHECK LIST
Please ensure you have looked over each item below.

Registration deadline is December 19, 2014. Registration is limited and will be accepted on a first
come, first serve basis. The fee must accompany the registration form. Registrations that are
incomplete at time of deadline, will be placed on a waiting list.

Total team fee is $450 for U13-16 (11v11). $385 for U11/12 (9v9) and $275 for U9/10 (7v7) - due upon
registration. HST tax must be applied to all divisions.Payments can be made by Cheque, Cash, Credit
Card (online or over the phone) or Debit.

Each team will be guaranteed a minimum of 3 games + playoffs, for teams that qualify (divisions U13+)
and all games will be officiated

Teams within the Hamilton & District Association do not require travel permits. All other teams must
complete an application to travel and submit the approved permit with their entry form (failure to
present a valid travel permit will result in forfeit of all scheduled games without refund).

11v11 U13-U14 will be 3 x 37 minute games, U15+ will be 3 x 40min games no half time. 9v9 games will
be 3 x 26 minute games, no half time. U9/U10 Boys & Girls games will be played 7v7 format (3 x 26
minute games, no half time). These tournament durations times are compliant with the OSA standards.
U13-U14 – 150min max, U15+ 180min max.

Email communications will go out to both Team Coach and Manager for updates on league, scheduling
or special announcements. Tournament schedule and information will be posted and made available
the week of December 22nd .

Teams are required to provide two sets of jerseys with numbers on the back of each jersey. Goalie
must wear a different colour uniform than the rest of team. No metal cleats allowed on field.

A team list must be provided and all players must be in possession of a player book stamped for the
2014-2015 indoor season (or outdoor stamped books for 11v11 divisions for the respective season),
SAAC Cards or equivalent for the tournament in order to be eligible to play in the tournament.

All games will be played at Players Paradise Sports Complex located at 565 Seaman Street, Stoney
Creek, ON (near the QEW & Fruitland Rd)

All team coaches, players and members agree to abide by the facility policies as set out by Players
Paradise Sports Complex
