Vancouver Board of Education (VBE) Student Application Form

Grade 8 Course Selection Form (for Grade 8 application only – not required for SPARTS
applications)

Student School Transfer Form (if currently registered at a Vancouver Public Secondary School but
not required for Gr.8 applications)

Term 1 Report Card

Valid Proof of Canadian Citizenship: Birth Certificate, Certificate of Citizenship, or Permanent Resident
Card

Proof of Residency (One item in EACH the following 3 categories is required: must be from a
recognized government office)

Purchase Agreement or Property Tax Statement

Hydro, telephone, or Cablevision Bill

Driver=s Licence, Vehicle Registration, Bank Statement, or Canada Post Mail Forwarding
Application
Cross Boundary Application Families may only accept one cross boundary offer. Once a family agrees to
accept an offer, their application for cross boundary at other schools will be nullified.


 Please provide copies of documentation required for registration, otherwise $1 per
copy will be charged for photocopying services.
 The address provided on the VBE Student Application Form should be the address
where the student is living as of the date of application.
 If your child is accepted elsewhere, please contact Magee (604.713.8200) ASAP to
cancel this application
