Household’s gross monthly income must be at or under 130% of the federal poverty guidelines. May qualify
even if income is over the limit if someone in the household is elderly/disabled. A review of your household
expenses is conducted to determine your monthly benefit amount if your income is under the limit or if the limit
doesn’t apply. Expenses reviewed include rent or mortgage costs, gas, electric, water, sewer, telephone, medical
expenses and payments for child support and child care. Applicants must provide a picture ID (valid driver’s
license, state ID or passport), Social Security card, income verification (i.e., pay stub) and verification of
