We are offering a $500 sign‐on bonus for all families successfully transferred. Childcare Reimbursement: The Bair Foundation will continue to pay the current rate of any children transferring to our organization including the childcare reimbursement. Approval of childcare for future placements is pending. Transportation Reimbursement: Mileage reimbursement is included in the per diem rate for the child. Transportation Provided by Provider: The Bair Foundation foster parents provide for the transportation of the children placed in their home; however staff and alternative care providers will assist with transportation if needed. 3‐28‐2012 Page 1 Provider requirements for childcare provider: The Bair Foundation will approve childcare providers to provide care for the children during the day. These providers are required to do the following:  BCII Fingerprints (paid by TBF)  FBI Fingerprints (paid by TBF)  Safety Audit of the home  Current copy of their driver’s license and auto insurance  BMW check  Training on HIPAA, discipline policy and confidentiality policy. These requirements are done annually. If the family utilizes a Type A or B daycare we require a copy of their ODJFS license. If a child stays overnight there are additional requirements. The Bair Foundation will assist in providing overnight care. Will provider accept FCCS babysitters to continue providing child care to FCCS Foster Parents? Yes, the Bair Foundation will accept FCCS babysitters to continue to provide child care. We will request documentation from FCCS verifying their approval. Any of the providers that are approved by FCCS to provide overnight care, we will request approval from FCCS to continue this use and authorization to pay those families for the overnight care. We require each family to be able to provide for their household without the foster parent per diem or stipend. The Bair Foundation requires all foster parents to maintain 60 hours of training (through classroom, online, books, videos, and mentoring), First Aid, CPR (adult, infant and child), Passive Restraint Training, Moves Training (annually), and Medication Administration (annually). The Bair Foundation requires attendance at the quarterly foster parent meetings where we offer training hours, reimbursement and time to meet and fellowship with other foster parents. Weekly foster parent logs to provide an update on the child’s progress, doctor’s appointments, visitation, etc. 2. Transport children to visitation, medical, and social activities 3. Maintain rule compliance at all times regarding the children’s medical appointments (medical, dental, vision, and psychiatric (if applicable) We require all families to have car seats available for placement of children between the ages of 0‐9. That includes infant, toddler, booster seats Foster Parent skills/characteristics Providers are seeking: The Bair Foundation is seeking foster parents that continue to have a love for children and their families with a willingness to continue to learn new things. We want families that will comply with the rules of the state (we understand there are a lot). We want families that will work with the team and bio families towards reunification or the best plan for the children. We want families that are open to collaboration and open communication with all parties involved. And lastly families with a dedication and a continued heart to foster abused and neglected children. Provider requirements for foster parents to transfer: The Bair Foundation requirements for transfer families are those of ODJFS: signed release of information and letter of intent; new BCII/FBI prints, 3 new references, interview with assessor/licensing specialist, current pet vaccinations, new safety audit, current driver’s license and auto insurance, First Aid, CPR, and Passive Restraint training within 30 days after transfer. 3‐28‐2012 Page 5
