Documentation and Coding Compliance
for Rehab Services
Saturday,
March 7
2015
Instructor: Holly Hester, PT, DPT, CLT
Description
As practicing clinicians are aware, medical review is
increasing. More and more often, payers are asking to review
medical records to ensure appropriate documentation of
services rendered to support billing and payment. This
course offers an in-depth review of regulatory and payer
requirements for documentation and coding, how to create
a complete and accurate “clinical picture” of the client
through documentation, proper CPT, and diagnosis code
selection to represent the services delivered.
Learning Outcomes
Participants will be able to:
• Demonstrate skill sets and knowledge of your
professional discipline through clinical documentation.
• Discuss how clinical documentation justifies
reimbursement and functions as a “receipt” for services
rendered.
• Discuss at least four specific requirements for
rehabilitation documentation.
• State at least 2 billing requirements for Medicare Part B.
• Demonstrate how to accurately record CPT and ICD-9/
ICD-10 codes for services rendered.
