APPLICATION PROCESS You may apply for up to three (3) high schools on the attached form. Be sure to apply for the correct grade level. Required credits are: Applying for an incorrect grade level may disqualify the application! 10th grade: at least five (5) credits (four (4) in required subjects) 11th grade: at least ten (10) credits (eight (8) in required subjects) 12th grade: at least 15 credits (12 in required subjects) Each year, students must earn at least five (5) credits, four (4) of which must be in the required curriculum. Applicants who apply after the deadline shall be placed at the end of their respective waitlist in the order they are received. Students currently enrolled in CCS may continue to apply through July 17, 2015. Students new to CCS may continue to apply until August 7, 2015 (transcripts must be received prior to this deadline). NOTIFICATION OF LOTTERY RESULTS Results from the electronic high school lottery drawings will be mailed to parents by mid-April. On or after April 17, parents may also call the FACTline (221-FACT or 221-3228) for results M-F from 9 a.m. to 3:00 p.m. Subsequent offers will be made through calls to phone numbers supplied on the application. RESPONSE TO AN OFFER If offered a seat, mark your decision by each school’s name on the result letter, sign and return it to the School Choice Division in person, by mail or fax (365-7468), no later than the deadline given in the letter. If no response is received, the offer will be cancelled and the next student on the waiting list will be offered the placement. Accepting an offer cancels all other applications. The seat at the previous or home school will be offered to another student, and your student will be removed from other waiting lists. If you decline an offer and your student is on waiting lists for other choices, those waiting list applications remain active. The last day that lottery offers will be made is September 11, 2015. Parents should contact the newly accepted school within one week of acceptance to determine next steps, i.e. meet with a counselor, attend an orientation, complete a schedule or determine summer readings. If you change your mind about an acceptance, your student is committed to the original accepted school until he/she has reapplied and been approved to attend another school, including the student’s home school by address. If a student is not placed before school reopens for 2015-16, the original assignment remains valid. Students should attend that school on the first day of classes. Students cross-enrolled and attending full-time at another site (such as CCS Virtual HS, DeVry, or Columbus Downtown HS) may only participate at the high school to which they are cross-enrolled. Please note that OHSAA has semester and age participation restrictions; also note that courses taken via CCS-VCAP may conflict with NCAA Eligibility opportunities. 6
