THE NON-HP GUEST MUST FILL OUT INFORMATION ON REVERSE SIDE OF FORM.
Guest requests must be submitted to the bookstore by April 29th for approval.
This applies to online and cash/check purchases.
A HPHS senior student may bring one guest who does not attend HPHS, provided that this form is completed in full.
No middle school students and no one over the age of 20 will be allowed to attend. In addition, the guest must
also present a picture ID (school ID, Driver’s License, state ID) upon entry to the Dance. Guests without acceptable
photo identification will not be allowed into the dance under any circumstances.
Highland Park High School reserves the right to decline the attendance request of any non-HPHS guest for any reason.
