INSTRUCTIONS:
To receive your mail-in rebate, follow these instructions.
1. Complete all information requested on this offer form.
2. Circle the qualifying Lexicon product purchase and the purchase
date on your receipt or invoice. Your receipt/invoice must
be dated between February 1, 2015 and March 31, 2015. Photocopies of the receipt/invoice are acceptable.
3. Remove the original UPC from the Lexicon product package.
Photocopies of the UPC will not be accepted.
4. Mail this completed offer form, a copy of the receipt/
invoice and the original UPC in a single envelope to the
address below. Submissions must be postmarked within
45 days of product purchase to qualify.
