You have reported a crime to the Omaha Police Department in which you lost property of value.
Oftentimes, only victims can positively identify their stolen property and there are seldom witnesses
to these crimes. Consequently, extensive cooperation between victims and the police is required to
recover stolen property.
It is very important that we have the best possible description of your stolen property. If the total loss
and a complete description of your property were given to the officer at the time your report was
taken, there is no need to mail in an additional statement of loss. However, we ask that you retain
this information form in the event that you need to provide us with additional property loss
information. When completing this form by hand please make certain that all information provided is
legible.
If you have photos of the stolen property please email the photos to the OPD Pawn Squad at the
email listed on the front of this form. Please include the RB Number (case number) on this form as
well as in the subject line of the email with the photos.
After the officer completes your initial report, the report will be forwarded to the Omaha Police
Department, Criminal Investigations Bureau. Your report will be reviewed and evaluated. If there is
sufficient information to conduct an investigation, a detective will be assigned to your case and will
