SUMMER TRAINING AVAILABILITY CONFIRMATION
This form is to confirm your availability to attend a summer training course this year. Below are
the available courses and dates. In order to confirm you are interested in attending, please follow
these steps:

Rank your 1st, 2nd, and 3rd choices.
Make sure you have only selected courses that you are eligible for.
Make sure you have only selected courses that you are available for.
Sign the form, confirming you have followed steps 1-4.
Ask a parent or guardian to review and sign the form.
Return the form to Lt(N) Lura or your Level Officer no later than 8 April 2015.
Once we have all completed forms, cadets will be ranked (by attendance, dress marks,
performance in training, etc), and a list of confirmed and spare/backup cadets will be generated.
Completion of this form does not guarantee that you will be attending training.
