The daily rate of $65 per child includes all excursion costs, admission
prices for outings, afternoon tea, transportation costs, and all program
sessions and activity consumables.
No G.S.T. is applicable.
Family rate fee discounts are applied via the Government Child Care
Benefit increased sibling % rate.
INFORMATION / CONDITIONS
VENUE: Adventure Zone operates from Room15 Mandurah Primary School, Hacket St Mandurah. Allexcursions depart from and return here.
ARRIVAL & PICK-UP: Adventure Zone opens at 6:30 am and closes at 6:00pm. Children must be picked up no later than 6pm.A $2 per minute fee applies after 6pmChildren must be signed in and outof the centre and the
Supervisor notified. Only people listed on the enrolment form will be allowed to sign children out when they are being picked up. Parents must accompany their children to sign them in.
EXCURSIONS: Children must be signed in before 8:30am.Returning no earlier than4:30 pm.
MEDICATION: If children require medication, written authorization must be given to the Supervisor, together with the medication and all relevant details.
CANCELLATIONS:Unfortunately no refunds for cancellations will be given during the holiday period.
PROGRAMME CHANGES: Due to unforeseen events or inclement weather it may be necessary for the Program Supervisor to change aspects of, and/ or modify the program at short notice.

Simply choose which days you would like your children to attend Adventure Zone,then complete the attached form and forward it with full payment to the address below. Remember to ring the FAO for your CCB % rate.
Places are limited for each day, so it is advisable to book early to avoid disappointment!
Parents must pay for the total bookings when enrolling.
Please Note:
Mail Bookings are not confirmed until receipt has been issued. Direct debit payable to Adventure Zone.
We do not accept Credit card or cheque payments.
Bookings are only confirmed with payment.

* I wish to enrol my children in the Adventure Zone Holiday Program as outlined in this brochure.
* I understand that Adventure Zone, its staff and management will take all reasonable care of my children and I will not hold them responsible for any damage and/ or loss of property and/ or accident. In case of accidental or
untoward incident I give my consent for any necessary medical treatment and agree to meet any expenses incurred. I realise that I am responsible for informing Adventure Zone staff of any medical conditions that may affect
my children’s participation in the program.
* I give consent for my children to be transported to outside venues as nominated by this program.
* I recognise that Adventure Zone reserves the right to remove a child from the program for any action by the child that may distract or hinder the program. No fee refunds will be given. This will include
