PLAYER NOTICE: Payment must be included with your order. Orders need to be turned into
your coach or at the concession stand no later than Wednesday April 22, 2015.
Pickup date is Friday May 1, 2015 between noon & 6 P.M. at the West State Street FIELDS. We
will not distribute any flowers early. Please make sure customer name is on order form and this bottom
portion. Customer should bring this bottom portion with them to pick up.
