 All players MUST have all registration and USA Hockey (Registration sheet with 20152016 USA hockey number listed, USA Code of Conduct and USA Consent to Treat)
submitted to NA Hockey prior to the try outs along with a check made payable to NA
Hockey for $ 150.
 The concussion testing and physicals will be required to be submitted to the NA
Athletic Director’s office prior to taking the ice for practice sessions in the Fall.
Players missing any paperwork will not be permitted to participate in practice sessions. No
exceptions. Additional information will be provided at a later date for concussion testing
dates.
 All players must be registered with USA Hockey for the 2015-16 season prior to
tryouts. Register online at www.usahockey.com, pay $45 fee, save and print confirmation
page and include USA Hockey number on the registration form. Confirmation page is not
required to be submitted.
 Players new to NA hockey are required to send a copy of their Birth Certificate with
registration. All 9th graders or new students to NA for the 2015-2016 season should submit
birth certificates.
 A non-refundable tryout fee of $150 made payable to NA Hockey is required with
registration. A $25.00 late fee will be assessed to anyone registering after May 10.

Players who are placed on the JV and Varsity teams should be fully aware that
playing for North Allegheny must be an absolute priority, and will be expected to attend
all practices and games unless there are special circumstances. The NA Coaching staff
recommends that anyone considering playing for both NA and another team check first
with that organization to ensure that it is “high school friendly” and will not require
players to miss high school games simply to attend their practices. If it is not, please
recognize that missing NA sessions will jeopardize the player’s status with our program.
