When Your Medical Leave Reaches 6 Months
Please review the following information to understand how your participation in the CarMax Benefit
plans change when your Medical Leave reaches 6 months.
Your participation in each benefit plan while on Leave is subject to the terms and conditions of each plan.
Please refer to the “Continuing Your CarMax Benefits While on Leave” overview or the Summary Plan
Document for each benefit plan for more information. All benefit plan information is available on the
CarMax Benefits website, available through the CarMax World or at carmax.com/benefits from any
computer with internet access.
This overview describes the changes to your benefits if you were deemed to be a participant in the plan at
the time your Medical Leave of Absence reached six months in duration and you had made all the required
premium payments to maintain your coverage during your Leave.

Please review the information provided below for all of the plans in which you are enrolled.
Medical, Dental, and Vision Plans
If you are enrolled in the Medical, Dental, and/or Vision Plans, your coverage will end on the last day of the
sixth month of your Medical Leave.
You and/or your previously covered dependents may elect to continue coverage under a federal act known
as COBRA (Consolidated Omnibus Budget Reconciliation Act of 1986) for up to 18 months. Approximately
14 days after your coverage ends, detailed information (including the cost and length of coverage) will be
mailed to your home address.

You and your dependents may also be eligible to elect medical plan coverage on the Health Insurance
Marketplace in your state of residence. Information about the Health Insurance Marketplace is available on
the healthcare.gov website.

Flexible Spending Accounts
HealthCareFlexibleSpendingAccount(HCFSA)
If you were enrolled and maintained participation in the HCFSA during your Leave, your participation will
end on the last day of the sixth month of your Medical Leave.
You will have 90 days following the end of the Plan Year (until May 29) to submit any expenses for
reimbursement that were incurred while you were a participant during that Plan Year. After this period, any
remaining balance in your account will be forfeited according to IRS regulations.
You have the right to continue participation in this program under COBRA for the remainder of the current
Plan Year (through February 28/29). If you elect to do so, you will be required to pay the full after-tax
monthly contribution plus a 2% COBRA administration fee. You will be billed each month you participate.
DayCareSavingsAccount(DCSA)
If you were enrolled in the DCSA, you will have 90 days following the end of the Plan Year (until May 29) to
submit any expenses for reimbursement that were incurred while you were an eligible participant during
that Plan Year. After this period, any remaining balance in your account will be forfeited according to IRS
regulations.
Life Insurance
BasicLife,SupplementalLife,SpouseorDomesticPartnerLife,orChildLifeInsurance
Any Life Insurance coverage in effect will end on the last day of the sixth month of your Medical Leave.
You or any of your covered dependents may be eligible to convert any lost coverage to an individual policy
with Aetna without having to provide evidence of good health. If you wish to convert any lost life insurance
coverage to an individual policy, you must apply for conversion no later than 31 days after coverage ends.

AccidentalDeath&DismembermentInsurance
If you are enrolled in Accidental Death & Dismemberment Insurance, your coverage will end on the last day
of the sixth month of your Medical Leave.
Disability Programs
Short‐TermDisabilityPay
Eligibility for Short-Term Disability Pay ends based on the schedule of benefits applicable to you as
determined by your tenure and Associate status. The maximum amount of any Short-Term Disability pay is
26 weeks.
Long‐TermDisabilityInsurance
If you enrolled in Long-Term Disability, your participation in the plan will end on the last day of the sixth
month of your Medical Leave. However, if you apply for and are approved for Long Term Disability benefits
based on plan requirements, your eligibility to receive benefits will remain in effect as long as you continue
to meet the Plan’s requirements and definition of disability. Conversion to an individual policy is not
available.

Other Benefit Programs
Retirement Savings Plan
If applicable, your participation in the Retirement Savings Plan will continue for the duration of your
employment. However, you will not be able to contribute to the Plan until you return to an active status and
begin to receive pay.
RetirementSavingsPlan(401(k))Loans
If you are on an approved Leave and you have an outstanding 401(k) loan, you will not be required to make
loan payments for up to 12 months as long as you either (1) receive no CarMax compensation while you
are on leave; or (2) your Leave pay (after tax withholding) is less than the amount required by the terms of
the loan. If you receive any pay while you are on Leave, loan deductions will be withheld from your pay
when possible.

suspended, you must resume payments when you return to work and repay the loan within the original
repayment period.
*Please note: You must be in an active status to withdraw a 401(k) loan; loans cannot be withdrawn while
you are on Leave.
Employee Stock Purchase Plan (ESPP)
If applicable, your participation in the Employee Stock Purchase Plan will continue for the duration of your
employment. However, you will not be able to contribute to the Plan until you return to an active status and
begin to receive pay.
Employee Assistance Program
You and your family are able to use Employee Assistance Program (EAP) services throughout your Leave
and for up to 18 months following your termination of employment. The EAP can be a valuable resource;
this free and confidential service is available by phone or online 24/7. Legal and financial consultations are
also available.
The EAP is available by calling Aetna at (866) 498-5004 or via the website at AetnaEAP.com and entering
the company code and password MYKMXEAP.
Tuition Assistance Program
Your eligibility for Tuition Assistance benefits will end on the last day of the sixth month of your Medical
Leave.
Adoption Assistance Program
Your eligibility for Adoption Assistance benefits will end on the last day of the sixth month of your Medical
Leave.
Tobacco Cessation Program
Your eligibility for the Tobacco Cessation Program benefits will end on the last day of the sixth month of
your Medical Leave.
Weight Watchers Program
Your eligibility for the Weight Watchers Program benefits will end on the last day of the sixth month of your
Medical Leave.

Associate Discount Program
Your eligibility for the CarMax Associate Discount Program will end on the last day of the sixth month of
your Medical Leave.
CarMax Rewards Discount Program
Your eligibility for the CarMax Rewards Discount Program benefits will end on the last day of the sixth
month of your Medical Leave.
The preceding information summarizes how benefits are impacted when an Associate’s approved Medical
Leave of Absence reaches six months in duration. If this information conflicts with Plan documents,
Company policies, and/or contracts, the Plan documents, policies, and/or contracts will prevail. If you have
