Parental Consent:
The named participant is in good health and has my permission to participate in Big Easy Lacrosse Club Summer Travel League 2015 (hence BELC). I hereby release BELC and the
affiliates and officials, and volunteers from and against any liability claims or demands for any injury or illness incurred at BELC 2015 during training, competition or travel to/from any
related event with any adult associated with the BELC. I hereby assume complete financial responsibility for any personal injury or property damage created as a result of an intentional or
negligent act of my child or ward while he is attending BELC. If my child needs medical treatment, and my consent cannot be obtained, I hereby agree that BELC may consent to appropriate
medical treatment for my child, including transport to a local hospital as deemed necessary. This will be in effect during all practices, tournaments & all associated travel, meetings, etc. I
will be responsible for any costs of medical treatment incurred at the camp during this period.

Publicity Release:
I grant permission for BELC to photograph or film my child’s name, image, likeness, spoken words, student work, and performance with or without associating names thereto and in any
form (hereinafter collectively referred to as “Works”), and to use, publish, display, distribute, produce, duplicate, sell, and copyright these Works in connection with any promotional
material that may be created by BELC. I waive any claim for compensation of any kind for BELC’s use or publication of the Works of my son. I hereby release, discharge, and agree to
hold harmless BELC and those acting under its authority from any liability to the extent provided by the law, for use or publication of the Works described above. By signing below, I
agree to the terms stated above.

Team Fees: $950.00 Fees cover over 52 hours training, all tournament entrance fees, and player rooms (4 per room).
