CONSENT AND AUTHORIZATION
I have carefully read and understand the Terms and Conditions for joining The Bone Marrow Foundation Team for the
2015 TCS New York City Marathon (Page 3).
If invited to join The BMF Marathon Team, I will abide by all Terms and Conditions. I agree to raise a minimum of
$3,000 for The Bone Marrow Foundation and I agree to raise this amount in full by October 31, 2015. I will provide
my credit card information, and I authorize The Bone Marrow Foundation to charge this credit card for any remaining
balance on October 31, 2015 if I have failed to raise $3,000 by this date.
I certify that my participation in the 2015 TCS New York City Marathon is voluntary and I will not hold The Bone
Marrow Foundation responsible for any injuries or adverse effects suffered by me in training for or participating in the
event. I also grant permission for The Bone Marrow Foundation to use my name and/or image in materials related to the
Foundation’s participation in the 2015 TCS New York City Marathon.

Runners requesting a guaranteed race entry on The BMF Team
Deposit and Fundraising Minimum
A runner approved to receive a guaranteed entry for the 2015 TCS New York City Marathon as a member of The Bone
Marrow Foundation Team is required to raise a minimum of $3,000 for The Foundation. Once the runner has accepted the
invitation to join the team, a non-refundable deposit of $25 will be charged to the runner’s credit card. The remaining
balance of the fundraising minimum must be submitted to The Bone Marrow Foundation by October 31, 2015.
A credit card (American Express, MasterCard, or Visa) valid through January 2016 must be provided in order to be
eligible to join The Bone Marrow Foundation Team. Once the runner has accepted the invitation to join the team, the $25
non-refundable deposit will be charged to this credit card. The deposit will count towards the runner’s fundraising
minimum. The credit card will not be charged again unless the required minimum has not been met by the October 31,
2015 deadline, at which time the balance required to meet the $3,000 minimum will be charged.
Cancellation
A runner approved to receive a guaranteed entry for the 2015 TCS New York City Marathon as a member of The Bone
Marrow Foundation Team may cancel participation for any reason (including injury) by contacting The BMF in writing.
Cancellation does not remove the runner’s obligation to fulfill the $3,000 fundraising minimum. However, a runner who
cancels participation and has fulfilled or agrees to fulfill the $3,000 fundraising minimum will receive a guaranteed entry
for the 2016 Marathon. (Runner must notify BMF by October 21st to be eligible). Any funds raised or fees and deposits
paid prior to cancellation or the decision not to run are not refundable.
Marathon Registration and Fees
A runner approved to receive a guaranteed entry for the 2015 TCS New York City Marathon as a member of The Bone
Marrow Foundation Team will receive an invitation with registration details from The Foundation after the runner’s
application is accepted. The runner will be responsible for additional processing and entry fees charged by New York
Road Runners through the TCS New York City Marathon. These fees do not count toward the runner’s fundraising
minimum. The processing fee for the 2015 TCS New York City Marathon through New York Road Runners is $11, and
the entry fee is $216 for New York Road Runners members (domestic), $255 for non-members (domestic), and $347 for
non U.S. residents.

Runners who already have a guaranteed race entry and wish to join The BMF Team
Joining Fees and Fundraising Minimum
A runner who has obtained a guaranteed entry for the 2015 TCS New York City Marathon through the lottery can request
to join The Bone Marrow Foundation Team and fundraise in support of The Foundation. There is no fundraising
minimum for runners who already have a guaranteed entry, but the runner will be asked to set a realistic personal
fundraising goal. A non-refundable $25 fee is required to join The Bone Marrow Foundation Team. Once the runner has
accepted the invitation to join the team, the $25 fee will be charged to the credit card provided on this application. The
joining fee will be counted toward the runner’s personal fundraising goal.
Cancellation
A runner who already has a guaranteed entry for the 2015 TCS New York City Marathon may cancel participation with
The Bone Marrow Foundation Team by contacting The Foundation in writing at any time. Funds raised before a runner’s
cancellation or decision not to run will not be refunded, nor will the runner’s $25 joining fee.
