 New Employee Checklist Employee Name ___________________________________ Please bring: Welcome Session Date _________________ ☐ Proof of licensure from ELAR (Teachers only) If your licensure is still pending at the time of your hire, it is your responsibility to supply OHC with documentation of your licensure by August 15 ☐ Letter of outside service (Teachers only) A maximum of 3 years of outside service will be granted. Must be on letterhead and include: 1) Dates of employment 2) That you were a licensed, full-­‐time teacher 3) That you taught at least 160 days per school year under contract ☐ Official transcripts Only sealed, hard copies of transcripts are accepted. Bachelor’s transcript(s) required for teaching, managerial, cluster sub, and BASAS positions. Paraprofessionals and CFCs must submit proof of associate’s degree or a minimum of 48 college credits. If applicable, please also bring graduate transcripts so that you are placed on the correct pay grade and step. A degree from outside the United States requires evaluation through CED (please allow sufficient processing time): http://www.cedevaluations.com/application_form.html. ☐ Social security card & another form of photo identification Social security card must be the official document, not a copy. If you do not have one, please be sure to write your account number legibly! ☐ Employee Withholding Allowance Certificate (W-­‐4) ☐ MA Employee Withholding Exemption Certificate (M-­‐4) ☐ State-­‐Boston Retirement System Beneficiary’s date of birth and social security number required ☐ Statement Concerning Your Employment in a Job Not Covered by Social Security (SSA-­‐1945) ☐ Health Insurance Notification If you choose to enroll, visit http://www.cityofboston.gov/ohr/benefits/group.asp for the enrollment forms and plan comparison chart. You will need your employee ID before signing up for health insurance-­‐ once you have received your ID from BPS, please bring your health insurance enrollment form directly to Boston City Hall or fax it to 617-­‐635-­‐3932 within 60 days of your hire date. For family coverage, you will need to send/bring your marriage and/or birth certificates as well ☐ Dental Insurance (Managerial only) Once you have your employee ID, please bring your dental insurance enrollment form directly to Boston City Hall or fax it to 617-­‐635-­‐3932 within 60 days of your hire date, along with your health insurance (if applicable) ☐ Life Insurance Notification Beneficiary’s date of birth and address required.
