2015 Brickhouse Classic Tournament Packet Inside this Tournament packet you will find the 2015 Brickhouse Classic Team Registration form, 2015 Tournament Waiver Form, and the Brickhouse Classic Code of Conduct. Please complete the Registration form and review the Waiver and Code of Conduct forms. A parent or guardian signature is required for each player who is participating in the tournament. This signature must be present on the Team Registration Form at the time of registration and weigh-in. ALL ROSTERS ARE FINAL AT WEIGH-IN. Team registration fee is $350.00 per team. PROOF OF AGE: A certified copy of the birth certificate on file bearing the seal of the issuing office of the state of birth is best guarantee of reliability of claimed birth (no mothers copies). Passports, Military ID cards, certified adoptions papers, certified wallet certificated issued by a state or commonwealth are acceptable. As lawful consideration for my child being permitted to participate in the Brickhouse Classic Youth Football Tournament, I agree that neither myself nor anyone associated with my child will make a claim against, attach the property of, or prosecute the Brickhouse Classic Organization for damage, personal injury or property damage which my child may sustain as a result of my child’s participation in this sporting event. This release is intended to discharge in advance all members of the Brickhouse Classic, its President, Directors, Staff and their agents, Sponsors, Facilitators, Photographers and Employees against any and all liability, including negligent actions, arising out of or connected in any way with the minor child’s participation in this sporting event except for the liability that may arise out of the willful and wanton misconduct of the Brickhouse Classic Organization. I understand that my child’s photograph may be taken during the Brickhouse Classic. I authorize these photos to be published online, in printed publications, websites, Internet, and any social media outlet. I understand that I will have the opportunity to purchase any photographs taken from Southern Belle Photography & Design. Signature ___________________________________________ Date _______________ 2015 Brickhouse Classic | Code Of Conduct To ensure everyone has a wonderful time during our tournament, THE BRICKHOUSE CLASSIC has created a team Code of Conduct Agreement to establish guidelines for coaches, players, and all team spectators in attendance. Our goal is to make sure that all of our participants will thrive in an environment where respect, team play, and camaraderie are practiced. SPORTSMANSHIP WILL BE ENCOURAGED AND ADVISED. Remember; THE BRICKHOUSE CLASSIC is a competitive tournament for classic matchups and the love of the game. Our Code of Conduct • Coaches will be responsible for the discipline of any INFRACTIONS within their own team. • Coaches are not to place the emotional and physical well being of their players ahead of their desire to win. • Abuse of a game official, coach, player, or spectator, whether it is verbal or physical, including but not limited to swearing, violence, or obscene gestures will result in removal of the individuals involved. • Game officials are the eyes of the field so please understand that they are there to make a call and not to honor an opinion of the sideline. The Head Coach shall be the only sideline negotiator when addressing an official, that way no information will be lost in translation. • Coaches or team officials must make sure the proper insurance is secure for each individual player or group policy. • Team Spirit and Chants are encouraged but cannot be used to taunt an opponent before, during, or after a game. Disrespect will result in immediate dismissal of the individual team from the tournament. • Head coach will take full responsibility that every player on the roster is within the age requirements of the division the team has entered. • Championship games will be followed by a trophy presentation and both teams must be present. The champions will celebrate and the runner-up will be acknowledged as we share this time in respect to our fellow competitors.
