Bristol Community Health Community Interest Company (CIC) became registered with
Companies House as a Social Enterprise on the 1st April 2011 with our Head Office
situated at South Plaza, Marlborough Street, in the centre of the city of Bristol. The
organisation commenced trading on 1st October 2011 and currently provides:
A wide range of Community Health Services across Bristol, North Somerset and South
Gloucestershire including:
•
Community Services for People with Learning Difficulties across Bristol;
•
Urgent Care Services from our Urgent Care Centre which is situated alongside South
Bristol Community Hospital;
•
Prison Healthcare Services at Her Majesty’s Prison (HMP) Bristol, HMP Leyhill, HMP
Eastwood Park and HMP Ashfield
•
Personal Care at Her Majesty’s Prison (HMP) Bristol, HMP Leyhill, HMP Eastwood
Park and HMP Ashfield
•
Diabetic Retinopathy Screening and Diabetes Education Services in Bristol, South
Gloucestershire and North Somerset
•
Continuing Healthcare Assessment and Review Teams in Bristol and South
Gloucestershire;
•
Palliative Home Care Service for people registered with Bristol and South
Gloucestershire GPs.
•
Community Nursing and AHP services
Bristol Community Health Services CIC works closely with Bristol City Council to provide
integrated services in intermediate care sites based in the north and south of the city.
Our staff also provide health services to disabled children receiving respite care and
support at New Belbrook residential facility which is a Bristol City Council facility
registered with Ofsted.

Bristol Community Health is classified as an independent sector provider in its registration
under the Health and Social Care Act 2008 and subsequent regulations in 2009 and
2010. Its regulated activities across services in each registered location are identified in
our CQC registration certificates and include:

‘Charters’ for Care and Customer Service which set out what patients can expect
from the organisation, supporting communities and growing partnerships.
Our Community Learning Difficulties Teams and Neuro Rehabilitation teams provide
specialist nursing and therapist support that helps to promote equal access to
mainstream health services for adults with physical and learning disabilities. The service
also provides direct nursing and therapy assistance to people using the service who are
visited in their own homes or in care homes. A limited service is also offered to children.
Examples of the assistance given to people and to their families include epilepsy
management, management of medication and support when people exhibit behaviours
that challenge others.
For people with a learning disability we operate an open referral system to GP-registered
patients, accepting referrals from health and social care professionals, carers and
service users themselves (subject to a service eligibility assessment if required). Our
teams are made up of the following professionals:

The organisation’s current revenue is £45.9 m with circa 1,200 staff, the majority of whom
are directly involved in patient care in community settings ranging from people’s homes to
our Urgent Care Centre. Our staff are specialist nurse practitioners, community matrons
and nurses, healthcare support workers, personal care assistants, GPs with a special
interest, consultant physiotherapists, physiotherapists, occupational therapists,
podiatrists, phlebotomists, diagnostics and learning difficulties teams combining nursing,
therapy and psychology services. The clinical teams work alongside their management,
clerical and administrative support workers from bases in health centres, clinics and other
sites run by our partner organisations throughout Bristol, North Somerset and South
Gloucestershire.
Healthcare is provided in partnership with others, including Bristol City Council, South
Gloucester Council and fifty-five local GP Practices. The population continues to rise and
projections show this is likely to continue across all age groups; furthermore the ethnic
diversity of Bristol also continues to broaden. Bristol Community Health helps support
local people to maintain healthy lifestyles in a home setting and to overcome illness so far
as is possible outside of hospital care. It continues to develop local services with our
partners to meet identified needs of the communities it serves.

Headquarters for all management, infection prevention and control, safeguarding,
governance, risk management, clinical effectiveness, library, human resource, training
and development, finance and administration activities.
Base location for Community Learning Difficulty Services in Bristol (registered for
Treatment of Disease, Disorder and Injury only) and Community Health Services,
Domiciliary Physiotherapy and Occupational Therapy, Diabetes and Nutrition Service,
Diabetic Retinopathy Screening, Diabetes Education, Palliative Home Care, Intermediate
Care and Re-ablement, Specialist Services in the community (e.g. Dermatology,
Musculo-Skeletal Outpatient Physiotherapy, Continence, Wound Care, Falls, Heart
Failure, Telehealth, Chronic Obstructive Pulmonary Disease, Infection Prevention and
Control), Palliative Home Care Support, Podiatry, Phlebotomy, Tuberculosis Nurse
Service, The Haven (specialist service for asylum-seekers), Rapid Response and Out of
Hours Services. Most services are accessed by adults and older people whilst some are
accessed by the whole population.

Function at our Urgent Care Centre:
Our Urgent Care Centre provides Urgent Care Services typical of a walk-in centre with
the addition of being able to refer for on-site X-rays and other diagnostic tests. No
appointment is necessary; patients can just turn up to during opening hours (currently
08:00-20:00 hrs). Nurse led assessment for conditions such as suspected broken bones,
diagnosis and treatment of minor injuries and minor illnesses (e.g. sprains and strains,
cuts and grazes and minor burns and scalds). The Urgent Care Service is accessed by
the whole population.
HMP Eastwood Park, Falfield, Wotton-under-Edge, Gloucestershire, GL12 8DB.
Function at HMP Eastwood Park:
Prison Healthcare and Personal Care Services are provided within this local closed
secure environment for remand and longer stay female prisoners.
Our teams deliver award-winning, nurse-led, prison healthcare and substance misuse
health support in partnership with Mental Health, Pharmacy and Primary Care providers.
Our 24-hour service provides prison healthcare services for younger adults, adults and
older females.
Babies up to twelve months old with their mothers in HMP Eastwood Park are under the
care of the GP practice.

Function at HMP Bristol:
Prison Healthcare and Personal Care Services are provided at this site, which takes
remand and longer stay male prisoners and is a Category B facility for the West of
England.
Our teams deliver high quality prison healthcare, pharmacy and substance misuse health
support services in partnership with Mental Health and Primary Care providers. The 24hour nurse-led service provides prison healthcare services for young adults, adult and
older males.
HMP Leyhill, Wotton-under-Edge, Gloucestershire, GL12 8BT
Function at HMP Leyhill:
Prison Healthcare and Personal Care Services are provided at this site, which takes
longer stay and Category D male prisoners.
Our teams deliver high quality prison healthcare and substance misuse health support
services in partnership with Mental Health, Pharmacy and Primary Care providers. The
Monday to Friday 8.00 am – 4.30 pm nurse-led service provides prison healthcare
services for adults and older males and also provides an ‘ad-hoc’ service should the
situation demand e.g. palliative care.
HMP Ashfield, Shortwood Road, Pucklechurch, Bristol, BS16 9QJ
Function at HMP Ashfield:
Prison Healthcare and Personal Care Services are provided at this site, which is a
Category C prison for adult males. Until July 2013, the site was registered as a Young
Offenders Institute before prison health services transferred to Bristol Community Health
CIC under new registration.
Our teams deliver high quality prison healthcare, personal care and health support
services in partnership with Mental Health, Pharmacy and Primary Care providers. The
nurse-led service provides prison healthcare services for adult and older males, 7 days
per week from 07.30 – 20.30 hrs.
