THE PERFORMANCE CENTER PLAN: The Golf Performance Center Development Plan offers players the following benefits until canceled by either the golf Member or the Performance Center. In consideration for the annual agreement, the player and any included family members listed below shall have the following privileges: (i) Professional Individual Instruction starting at ten dollars ($13) per half-hour lesson and twenty dollars ($23) per hour lesson. Limited to once per week per Member. Lessons may be booked one week in advance and are based on availability. (ii) Priority access to select clinics and tournaments. The Performance Center Membership is available for the golf Member and, for an additional fee, family members (social Members may join under nonmember pricing). We reserve the right to modify our program fees and benefits with 30 days’ notice. The Club is hereby authorized to make a charge to my Member Charge account, in the amount of $ per month. I understand that the monthly fee shall be pro-rated in the first month by dividing the monthly fee by 30 days and charging the remainder of the days not charged for that month. The Performance Center Player Development program is not available at River Forest or Healy Point.
