student, I release and hold harmless
Crossroads Christian Church (CCC)
from any and all liability, claims,
demands, and causes of action
whatsoever, arising out of or related to
any loss, damage, or injury, including
death, that may be sustained by the
student and/or the undersigned, while
in or upon the premises or any premises
under the control or supervision of
CCC or enroute to or from any of said
premises.

PHOTO RELEASE POLICY
CCC may take photographs of students
and artwork for use in publications
(print & email), exhibitions, our website,
and public relations purposes. No
compensation is provided to individuals
who appear in the photographs or for
artwork used. Registering your child
makes them eligible for such photo
release unless a letter is sent to CCC
requesting no photography of your
child.

THE FRUIT OF THE SPIRIT
Arts Smarts is an exciting week of
learning and fun that will introduce our
littlest campers to the diverse world of
the creative arts. The focus of the week
is to engage students in the creative
process which will often, though not
always, result in a finished product for
them to take home.
REGISTRATION
INFORMATION
Complete and return the attached
registration form by mail (PO Box 5386,
Evansville, IN 47716-5386) or bring it
to the Crossroads Christian Church
Office (10800 Outer Lincoln Avenue,
Newburgh). Please make checks payable
to Crossroads Christian Church.
You may also submit an online
registration and payment at cccgo.com.
(For campers entering grades 1 - 5, please
use the elementary registration form.)
Campers will receive their class schedules
and class locations by mail before the
first day of camp.
All registration forms and fees must be
received no later than Sunday, June 28.
We CAN NOT accept campers after
this date!
