Illawarra Shoalhaven Local Health District Implementation of Stage 1 of the NSW State-wide Eating Disorders Plan 2013-2018 Ms Paula Hakesley Dr Rajiv Singh Director of Mental Health Services Clinical Director, CAYMHS February 2015 Goal of Project To improve consumer experience through safer, more responsive, timely and accessible service provision for People with eating disorders in the Illawarra Shoalhaven Local Health District 1 Background & Aims The NSW Service Plan for People with Eating Disorders 2013-2018 has determined that the implementation of the Statewide Service Plan will be a staged process. Stage 1 (going through to June 2015) requires each Local Health District within their geographical boundaries to: – Identify current access points for services for people with eating disorders; – Identify major gaps in the service spectrum and workforce capacity to treat eating disorders; – Develop a Local Service Plan that will guide current and planned service provisions through to 2018 and beyond. The Statewide Service Plan identifies the LHD as the ‘central hub’ for health care for its residents. In keeping with the above priorities the Illawarra Shoalhaven Local Health District (ISLHD) Mental Health Services are facilitating the implementation of Stage 1 via the development of a Local Service Plan. Problem Identification ISLHD Mental Health Services quickly identified that not having a Local Eating Disorder Services Plan within our region had exposed us to the following gaps/weaknesses: o Lack of cohesiveness and cooperation between services o No service and/or professional networking o No specialist workforce plan o No local public health awareness and promotion programs The ISLHD region definitely required a Local Eating Disorders Service Plan 2 High Level Flow Detailed Level Flow Forum Focus on networking, identifying current service delivery components; strengths & gaps Release of NSW Client comes into Service prisonPlan eating disorders 2013-2018 Decision to organise a Closed Forum on 15.10.2014 Planning for the Forum LHDs required to produce Local Service Plan Analysis of feedback & output from Forum ISLHD identified does not have a ED Service Plan Project Scoping Paper developed for Executive sign-off Local Health Plan for Eating Disorders completed Terms of Reference written up Constitution of a Project Committee Project Scope defined Communication to key/peak bodies about Project Current status 1. Workforce Development Group Setting up of 3 Working Groups (reporting to Project Committee) Working Groups to provide action plan for further development of Local Service Plan Invite participation Planning & Implementation The development and finalisation of a Local Service Plan will be established using the following methodologies: Development of the vision for Eating Disorder Services. Conduct a gap analysis and identify data sources via a forum and working parties. Identify priorities for Eating Disorder Services development. Identification and development of access points. Development of Pathways to Care and evidence based treatments. Identifying workforce capacity building, education, training and support. Identification of Eating Disorder treatments/services within the geographical area of ISLHD. Clear pathways to specialist tertiary services outside of ISLHD geographical area 3 Project Results Scoping Paper priorities communicated to peak bodies. Project Committee constituted with wide representation from Paediatrics, Dietetics, Mental Health, Emergency Department, Consumer and Family Carer, Medicare Local, Statewide Eating Disorders Coordinator. ‘Closed’ Forum on Eating Disorders organised at WIN Stadium, Wollongong on the 15 October 2015 o 58 people attended o Representation from a wide range of services and backgrounds, including from outside the LHD Results The Eating Disorder Forum: Presentations from Statewide Eating Disorders Coordinator Director Mental Health and the Clinical Director CAYMHS to set the stage. Interactive patient journey mapping sessions based on specially developed case scenarios – to identify services, strengths and gaps World Café-style session focusing on solutions and other ideas 4 Eating Disorder Forum: Possible Solutions An analysis of the solutions and ideas revealed the following grouped themes: • • • • Early Identification Capacity Building Access to Information Improving Shared Care Strategies for Sustaining Improvement Based on the feedback 3 Working Groups were set up: o Strategic Service Development Group o Policies & Protocols Group o Workforce Development Group. All Forum participants were invited to be on the 3 working groups. A very good response was received, including a number who were unable to participate but were keen on contributing electronically. The Working Groups provided a final report and proposed action plan to the Eating Disorder Project Committee on the 30 January 2015. 5 2 Early Identification and Access to Treatment Introduce a screening tool to identify people that may be developing an eating disorder or have an eating disorder presenting to mental health services, headspace, general practitioners, school counsellors, emergency departments and other health services. Actions: – Literature review and selection of screening tool. By: 28/3/15 – Pilot and evaluate the screening tool at CAMHS and Headspace. By: 31/12/15 – Rollout the screening tool and pathways to treatment to extended services. By: 31/12/16 Establish an early intervention program at Headspace with pathways to escalate treatment to Child, Adolescent and Youth Mental Health Service (CAYMHS), Illawarra Eating Disorders Service (IEDS) and Shoalhaven Body Image and Eating Behaviour Service (SBIEBS) when required. Actions: – Review of evidence based therapies and programs for early intervention. By: 28/2/15 – Pilot an early intervention program at Headspace. By: 31/9/15 – Evaluate the program. By: 31/12/15 6 Plans to sustain change The Project Committee will continue to monitor and drive ongoing developments. The Working Groups will provide action plan for ongoing work in relation to further development and implementation of the Local Service Plan. The Local Service Plan will be the driver for continued implementation of the Stage 1 of the Statewide Service Plan (and will provide the basis for work and development for the next stages). Strategies for Spreading Present finalised Local Eating Disorder Plan to the ISLHD Board for sign off and to gain support and approval for both local media and general public presentations and discussions. In view of the strong interest and suggestions by participants in the Local Forum, there is a plan to organise a second Forum during 2015/16. One of the positive results of the forum was the establishment of a large data-base of interested clinicians/professionals from a variety of backgrounds which has enabled and encouraged networking. 7
