Addressing Social Determinants of Health

CQC is pleased to present a half-day discussion and
interactive event focused on social determinants of
health. What are social determinants of health, and
how does quality improvement work relate? Dr. Rishi
Manchanda, physician, author and founder of
Health Begins leads this event, setting the stage as
to the definition, role and importance of social determinants of health. Next, a
healthcare leader from an integrated health system in Northern California will
speak about a project in using data to address social determinants of health. Then,
Dr. Manchanda will illustrate the connection between quality improvement and
social determinants of health, presenting available tools, resources and databases
for audience members to take away practical application.
Finally, Megan O’Brien, MA from Health Outreach
Partners will address how to form and foster collaborative
partnerships among multiple stakeholders – including
clinics, health plans and medical groups.
Join us for this forward-thinking event meant to stimulate
conversation and inform innovative healthcare leaders of
current trends and ways to consider adjusting to meet with the changing times.

Rishi Manchanda, MD, MPH | Physician; Founder, Health Begins; Author
Dr. Manchanda is an "upstreamist." A physician and public health innovator, he aims to reinvigorate primary care
by teaching doctors to think about—and treat—the social and environmental conditions that often underlie
sickness.
Megan O’Brien, MA | Health Outreach Partners
Ms. O’Brien provides training, technical assistance and program support that focuses on expanding and improving
health outreach programs at community health centers and other community based organizations nationwide.
