DECLARATION, WAIVER, RELEASE, AND
HOLD HARMLESS AGREEMENT: Upon signing/
submitting this tournament application, I hereby
waive and release all other contestants, sponsors,
City of Wellington, and tournament officials from all
claims of death, personal injury, accidents, theft and
damage to personal property in connection with this
tournament. I understand there are possible dangers
to myself, my partner, and all equipment involved in
fishing. I assume full and all responsibility for my
health and safe being. I assume responsibility for all
equipment on and off the water during the tournament. I have sufficient health, accident, and liability
insurance. If I have no insurance, I represent that I
am capable of paying for any and all such expenses
or liability. I have read and understand the rules of
this tournament. The City of Wellington Lake has
permission to use my name and/or photograph if
they choose to do so. Having read and fully understand this declaration, waiver, release, and hold
agreement, each participant agrees by the above
stated terms and conditions as shown by the signatures below.

Official Rules and Regulations
RULES - It is each contestant's responsibility to read and fully understand these rules as entrants will be
subject to disqualification for noncompliance and forfeit all prize money and entry
fees. All entrants agree by signing this entry form that the decisions of any presiding
tournament official will be final in all respects.
ELIGIBILITY - This event is open to any 2
-person team paying the entry fee. One
team member must be 18 years of age.
ENTRY FEE - The entry fee for this event
is $50 per team. Entry form shall be completed in full and signed by both team
members. Entry fee must be paid in full at
time of registration.
REGISTRATION - Pre-registration will be
at City Hall before May 14, or you may
register May 16 from 6:00 to 7:00 a.m. at
the Wellington Lake Maintenance building.
SPORTSMANSHIP - Any contestant who
displays poor sportsmanship, violates any
tournament rules, or violates any local or
state game laws may be disqualified at the
discretion of tournament officials.
SAFETY - Each contestant is solely responsible for his or her own safety. State
safety laws apply.
WEIGH-IN - Weigh-ins will be at the end
of the fishing day. *Fish must be alive
and releasable, no dead fish will be
weighed. Only walleye and wiper fish
will be eligible. All fish must be caught
from the Wellington Lake from the hours
of 7 a.m. - 3 p.m. May 16, 2015.
LIMITS - 18" length limit on all fish,
with max of five fish caught each day per
person.
LICENSE - Must have a State fishing license.
RESERVED RIGHTS - Tournament officials reserve the right to accept or reject
any or all entries for any reason. Interpretation of the rules is subject to the sole
discretion of the tournament officials.
Tournament officials reserve the right to
refuse or withdraw the entry of any person if there is a reason to believe such entry might endanger the safety of any person or cause disruption of the tournament.
