PURPOSE
The Nurse Practitioner (RN(EC)) provides primary health care with a strong emphasis on
health promotion and disease prevention within the scope of practice and standards of
care outlined in the documents below. The NP diagnoses and manages common health
problems and diseases both independently and in collaboration with the client and the
physicians. The NP works as part of an interdisciplinary team to address issues in the
community that affect health.
ACCOUNTABLITY:
The NP is to practice within the scope and according to the standards of practice as
outlined in the following documents:

DUTIES AND RESPONSIBILITIES
Provisions of Primary Care
• Provides primary care within the scope of practice of Nurse Practitioner as outlined in the
College of Nurses of Ontario’s - Standards of Practice for Registered Nurses in the
Extended Class. The provision of primary care includes: assessments, diagnosis,
counselling, screening, referral, education, treatment and follow-up.
• Provides care to well individuals of all ages and their families, placing an emphasis on
health promotion and risk reduction.
• Collaborates effectively with physicians to facilitate appropriate provision of primary care
outside of scope of practice.
• Supports clients and families to take increased responsibility for their health by involving
them in risk factor and health problem identification, goal setting and the choice of
interventions for disease treatment, prevention and health promotion.
• Determines the need for, orders and interprets screening and diagnostic laboratory tests
chest and limb X-Rays and diagnostic ultrasounds.
• Maintains complete, accurate and timely records of client visits, which will facilitate the
appropriate provision of care by other health professionals.
• Refers clients appropriately to comprehensive health programs of FHT as well as other
Community Partners.
Community Development
• Monitors demographic and health trends within our community and develop plans to
address community health needs.
• Partners and link with community organizations and groups to enhance access to primary
care for clients.
• Possesses knowledge of special health needs, risk factors and barriers to health of
population groups in our region.
• Possesses knowledge of health and social services available to clients of the Family Health
Team either within the FHT or through other services/agencies within and outside our
region.
• Synthesizes information from individual clients to identify broader implications for health
within the family and the community.
Health Promotion and Disease Prevention
• Participates in planning, implementation, and evaluation of community health programming
for target populations.
• Implements strategies in partnership, to promote health and prevent disease with
individuals, families and groups.
Reporting
•
Prepares statistical reports for the CKLFHT and Ministry of Health and Long-Term Care.
Team Development
• Works effectively within an interdisciplinary team, in varied practice settings, to improve
access to primary care for the residents of the City of Kawartha Lakes. The “client” may be
an individual, a family, a target group or a community.
• Promotes awareness of the FHTs services and programs
• Actively participates in staff, team and committee meetings as appropriate.
• Provides consultation as requested to community groups, partners and team members.
• Provides leadership and mentorship to nurse practitioner and nursing students that enter the
FHT on a placement. Participates in the education of other health professional students.
• Participates in the development, planning and evaluation of treatment, education,
counselling and health promotion activities of the FHT.
Professional Development
• Maintains and develops professional competence through ongoing professional
development. Fully participates in the “Reflective Practice” of the College of Nurses of
Ontario.
• Stays current and aware of opportunities to implement new, evidence-based methods of
client assessment and treatment.
• Participates in self-directed learning to ensure that practice remains relevant by attending
professional conferences, e-learning and journal reviews.
• Participates on interdisciplinary committees to promote professional / interpersonal
development.
• Participates in clinical projects / studies as required.
Communication
• Communicates effectively with health care team members to create a cohesive team and
seamless services to the community.
• Communicates effectively with all clients, families, peers, other health care professionals
and community partners.
• Participates in interdisciplinary meetings as required.

SPECIFICATIONS
EDUCATION / EXPERIENCE:

Baccalaureate or Masters in Nursing.
Successful completion of post-graduate Primary Care Nurse Practitioner program.
Current extended class registration with the College of Nurses of Ontario.
Current membership with RNAO, including professional liability insurance program
Basic CPR certification.
Current Ontario Drivers’ License.
Demonstrated experience in Community and/or Primary Care Setting.
Demonstrated experience in the area of chronic disease management and prevention.
SKILLS / ABILITIES:
Knowledge and understanding of:
• Canadian and Ontario Health care system.
• The role of the NP within an interdisciplinary team, how to optimize collaborative
relationships.
• The patient as an individual, a family member and a member of the community.
• Issues specific to the residents of the City of Kawartha Lakes and resources available.
• Knowledge and proficiency in current, evidence-based methods and practices of
primary care, with an emphasis on heath promotion and risk reduction.
• Superior leadership, organization, research, evaluation, time management,
communication and interpersonal skills.
• Proficiency in the use of the computer hardware and software, particularly in
Microsoft Word, Excel, Outlook.
• Desire and ability to update knowledge and skills through various means including
technology-based opportunities, courses, workshops and conferences.
• Multi-tasking skills related to the delivery of efficient primary care including the
ability to share information and teach while treating a patient.
• Excellent verbal and written communication skills.

PHYSICAL DEMANDS:
Moderate: The position requires minimal physical effort and the workday comprises
roughly of equal amounts of standing and sitting, with frequent position changes. This
position may involve routine exposure to persons with contagious or infectious illnesses
and requires constant attention to infection control procedures.
MENTAL DEMANDS:
The RN(EC) must be able to make decisions based on the best information and evidence
that is available. An ability to exhibit critical thinking and deductive reasoning is vital.
The RN(EC) will have competing priorities and must be able to prioritize and balance
many tasks and responsibilities. The position may involve occasional exposure to highly
emotional or violent patients. The RN(EC) may occasionally need to relay upsetting
health news to patients or to deal with those who are terminally ill.
FINANCIAL RESPONSIBILTY:
Participates in planning of a set budget. May participate in the expenditure of resources
as planned.
IMPACT OF ERRORS IN JUDGEMENT:
Poor decisions and/or omissions can impact harmfully on client health. The level of
impact could range from very low to extremely high.
Poor interpersonal skills could impact negatively on relations with staff, community
partners, clients and public. The level of impact could range from very low to moderate.
WORK ENVIRONMENT:
FHT Clinic – City of Kawartha Lakes
The work environment will primarily be in well-lit, well-ventilated clinic areas that are
furnished ergonomically. Exam rooms are spacious, well equipped and supplied.
Personal office space may or may not be shared.
FHT Satellite or outreach sites
Environment may vary depending on location. Ergonomics, health and safety of the
RN(EC) will be an important consideration in how the location is furnished and
equipped. It may be required to bring some requisite equipment and supplies when
reporting to work in these environments.
Other
Travel may be required between sites and client homes. Travel related to the fulfillment
of this position description will be reimbursed.
REPORTING RELATIONSHIPS:
The Nurse Practitioner reports to the Executive Director. In clinical matters, the Nurse
Practitioner collaborates with the primary collaborating physician, family physicians and
other FHT members to provide the most efficient, appropriate primary care to a client of
the FHT.
