Step 1: Apply to the ISU College of Graduate and Professional Studies Create your online applicant profile at www.indstate.edu/graduate/apply.htm. Please note that there is a fee for this application. When prompted, select the following options: Level Type: Graduate Student Type: New Graduate Courses: Indiana State Online Program: Doctor of Nursing Practice Appropriate Semester Failure to complete the online application correctly will delay and possibly disqualify you from consideration for admission. The following items will be submitted only to the College of Graduate and Professional Studies: ____ Official Transcripts: Official transcripts for all institutions attended (undergraduate and graduate) must be sent directly from the school attended to Graduate Admissions. U.S. Postal Service: Graduate Admissions, Indiana State University Welcome Center, 318 North Sixth Street, Terre Haute, IN 47809-1904. E-transcripts may be sent to: [email protected] Foreign College and University Transcripts will need to be submitted with official evaluations. ____ Three letters of reference (including at least one academic and two professional references). These letters will be requested via the online College of Graduate and Professional Studies application, Supplemental Materials/+ add a recommendation request (An email will be sent to the recommender from Indiana State University with a link to upload there letter. Please let your recommender know to watch for the email.) Processing of these items can take 3-5 business days once they are received. All items must be processed by the deadline dates. (Feb 1, Sept 1) Step 2: Submit Application Materials to the Department of Advanced Practice Nursing (hardcopy) and upload all documents to the College of Graduate and Professional Studies on-line application. Mail your completed hardcopy packet to the address on page 2. You must also upload all documents to your College of Graduate Professional Studies Application via the Supplemental Materials section. Complete packets must be received no later than noon EST on Feb 1 or Sept 1. Your packet must contain the following items: ____ Completed and signed department application ____ Completed and signed department application ____ Completed Application Checklist ____ Government issued photo ID and a 2x2 color photo ____ Résumé or CV, including all employment history ____ DNP Essay/Goal Statement describing your area of interest (e.g. advanced clinical practice, administration, education, etc.) and a potential clinical problem that you plan to focus on as your identified topic in the Doctor of Nursing Practice program of study. Include why you are interested in this problem, why it is relevant for the current society, why it potentially warrants practice change, and how it is relevant to the role of the DNP graduate. ____ Sample of Scholarly Writing ____ Proof of current CPR certification for healthcare provider (copy of front and back of card) ____ Verification of a current, unencumbered Registered Nurse license or equivalent, in the geographical location where you will be doing your preceptorship or clinical practice. This can be found at your state’s e-verification website. This will include your name, license number, expiration date, current status, and any disciplinary action. As a student in the College of Health and Human Services – School of Nursing you will be expected to adhere to the policies and guidelines found in the Handbook. This can be found at: http://www.indstate.edu/registrar/Immunizations.htm. This can be completed online or mailed to the Office of Registration and Records. Please make sure this record is completed in addition to the Nursing Immunization Record. DO NOT include this form in your packet to the department. The DNP Program allows up to 12 credit hours that can be transferred (if determined equivalent) into the program. This will help us in processing your application quickly and ensures that all required document materials are received by the deadline. Please make sure to submit items to both the department via a mailed complete packet and also upload all documents to the College of Graduate and Professional Studies online application. The APN Department recommends submitting all documents, and completing the online College of Graduate and Professional Studies Application, at least one month prior to the deadline. Failure to submit materials correctly and by the deadline date will disqualify the applicant from consideration of admission.
