We are a FOLKSTYLE club and strictly concentrate on improving your skills to focus on making you better for your HIGH SCHOOL or JUNIOR HIGH TEAM Wrestlers will develop solid technique and competitive mental toughness on the mat, along with the confidence, self-discipline, and strong work ethic needed to succeed in life. The coaching staff specializes in throwing techniques and takedowns. All Coaches are welcome to come and help Please be on time for class and ready to wrestle at the start of practice. Signature to this agreement certifies that to the best of Member(s) knowledge Member(s) does not have any physical limitations that would prevent Member(s) from participating in the Judo Club’s activities, is fit and qualified to use the services and facilities of the Club and Member accepts the responsibility of Club membership subject to the terms and conditions set forth below: There will be no refunds of quarterly, yearly, summer or winter camp fees. The Club Director shall have the sole discretion to decide whether and under what conditions credits may be issued. RELEASE OF ALL LIABILITY AND INDEMNIFICATION AGREEMENT In consideration for the services which you receive as a member of Cohen Brothers Training Center, I/we do hereby for myself/ourselves and for my/our minor children, my/our heirs, executors, administrators, and successors, assign, release and forever discharge Cohen Brothers Judo, Ultimate Fitness, North Shore Baseball Academy, LLC, 400 Corporate Woods Parkway, LLC & Powerlink Electric Inc. (Hereinafter “Group”) its officers, directors, and agents from any and all losses, claims, demands, actions, suits at law and in equity, judgments, and executions which I/we had, now have, or may have in the future, arising from or by reason of any and all bodily or personal injuries whether known or unknown, death, and/or property damage resulting from, in connection with, or arising out of social events, social gatherings, social functions, Judo instruction, Judo tournaments, travel to and from Judo tournaments, Judo activities, and any and all other sports activities and any other activities of any kind or description in any way connected with the Group, whether sponsored in whole or in part by the Group. I/we acknowledge that the sport of Judo is a dangerous activity that can result in personal or bodily injury, including but not limited to death. Furthermore, in consideration for the services I/we will receive as a member of Cohen Brothers Judo Club, I/we on behalf of myself/ourselves individually and on behalf of my/our minor children listed above and their heirs, executors, administrators, or assigns, hereby agree to and will indemnify the Group, its officers, its directors, and its agents, and hold them harmless from any suits, filing fees, costs of collection, and attorney fees. I/we have read the foregoing APPLICATION AND RELEASE OF LIABILITY AND INDEMNIFICATION AGREEMENT and understand all of the terms thereof. I/we execute it voluntarily and with full knowledge of its significance.
