Each participant will be expected to adhere to all camp rules and regulations. Noncompliance is grounds for
dismissal from camp.

In the event that my daughter is injured or beco mes ill while attending the Bearcat Volleyball Clinic, I give my
permission for the staff to seek medical attention if deemed necessary under the existing conditions. I release
Cullman Bearcat Volleyball, the coaching staff, the trainers, and Cullman City Schools from any claims from
injuries sustained during the camp. I also certify that my daughter is in good physical health and that she will
notify staff members of any conditions that may impair her ability to participate in all camp activities.
